Link to entry by barcode

I need to create 2 libraries.. one holding the information about a book, and the other holding a list of notes. While reading a book, I want to be able to create an entry in the notes library to add some thoughts about what I am reading, scan the book barcode, and link that note entry to the entry holding the book details in the other library.  Is this possible and if yes, can someone please tell me how to set it up?
