What are hospitals using for their downtime forms?
Are you maintaining both the NUR screens and a paper form?
How did you create your downtime forms?

I use a report I got from someone on the L list to print a blank CDS.

So I printed a copy of the main screens we use daily (ADMIT
HX/ASSESSMENT,SHIFT ASSESSMENTS,VITAL SIGNS,NOTES, ETC...)

We sent these downtime forms out to the NURSING UNITS and they are
suppose to make copies as needed and keep an original for future use.

I keep a copy and the Nursing supervisor keeps a copy.

I also downloaded these forms as text files to my computer.

Originally, our downtime forms were the forms that we used before NUR. This worked well for a while, but as time goes on and nurses come and go, you find that most of the nurses have never used the original form and may not understand the complexities of it. Also, over time you tend to add new documentation requirements and delete others, so the original forms are no longer relevant.
Since downtimes are rare and typically not very long, we now have very few downtime forms and the rest of the information is documented in narrative notes. We have specific forms for weight/vital signs (simplified from the original version that had graphs and was used for several days, a blank MAR, an IV record and order requisitions. The admission and shift assessments are the only forms we have to keep up to date on paper and they are revised yearly.

We create a profile with a snapshot of the screen we want as a downtime. We then use the print report routine, pull up the profile and download it and open as a Word document. We then edit the headers and such out and for group responses that are not to long , add the choices so the user can just circle it. A paper copy is kept in the Nursing office and the forms are available to print from our Intranet

Lorraine, we have Access Eforms forms generation software. You can print the forms populated with Meditech data from Meditech (uptime use) AND print the forms blank before/during downtime. It also has the ability to admit patients in Access during downtime (AT point of registration), and print the forms OUTSIDE of Meditech, thereby allowing forms to remain the same, with patient data printed on them, no hand written stuff.
You can then manage the data input from the Access admit database back into Meditech (scripting works here if you wanted to).
This is assuming the network is up, just Meditech is down.

We have had them for over 3 years, and it has been a VERY successful implementation. (I could fill this post up with all the other very cool stuff it does, but that was not the point of the post)
