Good Afternoon L,

We're considering a Quest Laboratories interface. Are there any
Meditech C/S sites that have a SORR (Send Orders Receive Results), a
ROSR (Receive Orders Send Results) or both interfaces with Quest? If so
would you be able to discuss the length of time it took to implement,
challenges you identified in that process and your satisfaction with the
interface(s)?

We are C/S 5.55 and use Quest with a Medinet interface. (We got QUEST to pay for the interface). It works well, and it didn't take long to get the interface up and running, but we did have problems with our side dropping the connection periodically. That was solved with a script. It does take time to get all of the tests built, but of course that would be true of most any reference lab. One little tidbit of advice: ALL QUEST tests should be built as "Numeric? N".

Hi Teresa,

Good job on getting Quest to pay for your interface! Thanks for
responding. We had a Medinet interface with Quest years ago that
required a Modem box that sat on the counter top that had to be toggled
on and off. Is that how you do it as well or are you operating with HL7
messages? What does your script do to keep the connection up?

HL7 messages. They travel through the MIS IN box/OUT box and we result them in an analyzer process screen. I don't know the details of the script, but I'm pretty sure our Systems Administrator would share if you decide to go that route.

Yes, all tests need to be made non-numeric. They have a lot of "See
Comment" results. We are on the same system C/S 5.55. Quest Chantilly
does everything as a panel with every possible "T" test known to mankind
in it. That was the hardest part.

We are C/S v5.64 and implemented a non-discreet Medinet to
Quest.......mainly so we did not have to rebuild over 500 tests. All are
T-tests non-numeric. Our only issue with this is reflexed tests. We get
a fax from them anytime a reflex is added (as in an ANA titer from a pos
ANA screen) and there is no way to result the reflexed test without
going in and manually ordering it......then resending the failed INBOX
message. None that I have discovered anyway. They dropped me on
this.....said it would be no issue when we fleshed it out and it is....a
big issue.

It has taken me alone so far 208 hours........then another 75 for our
processor who helped secure the REF RES codes.

I have built a Quest SORR interface with C/S 5.5 and was able to get my ANAs to reflex. What I did was build the T-Type test non-numerically and use result codes for Negative and Positive. Then I built an Order Group for the Titer and Pattern, attaching this to my result groups to reflex of the Result of POSITIVE. Works like a charm, unless the titer ends up being <40 (or whatever they report as negative). In this case, I just need to go in and NP the Pattern result, as there is no pattern. With the reflex built, it automatically orders the required tests. I also have the Analyzer Type set to auto release and File W/O review the Repeat and Confirm. How do you get MT to make a determination of when to reflex? ALL our
results are entered as a dot ( . ) by Quest and all the actual results
are entered as a comment. We never have anything but a dot........so
there is no way for us to evaluate for a reflexed order group. Is your
interface a discreet interface? We are non-discreet.....only one OBX
with a dot as the result and the actual results as a comment to that
"dot" result.

The easiest way to avoid this is to do what you thought was too much
work up front. You should build new tests and make all your orderable
tests G or P type tests.

You then create profiles that includes the reportable T tests in them.
This allows you to include every single possible reflexed result under a
single order. Many of those tests will need to be auto NP'd as you may
not always get results for them. It is also fairly easy to change the
profile when your reference lab changes their reflex rules.

We have been interfaced via Medinet to Quest since they were SmithKline
labs. Anyway, we have tests set up as numeric because that worked for
us. Some can be set up that way. Having stated that, if I were
starting new on this interface, I would make them all non-numeric for
the reasons stated. We are Magic 5.62.

We have a Labcorp NMI (SORR) interface with Meditech (C/S 5.55). We are connected through a VPN connection. I have built several reference lab interfaces in the past and each time I learn something that I wish I had done differently.

One thing: make all of your tests numeric "N" and make all of your orders a Profile, even if there is only 1 test to result. You can have a profile with 1 test in it. When there are possible reflexes, put these in your profile with NP as the auto-result. If they reflex, you can write a calculation to add on an order group which will have a reflex bill code in it. If the reference lab makes a change to their test profile, all you have to do is add/delete tests from your Profile and your orderable mnemonic never changes. It makes it much easier when you are trying to maintain the interface.

I also like to make different departments for the "room temp," "frozen," "refrigerated," and "ground ship" specimens. Even though you can sort your site batch by temp, it makes it easier on the techs and couriers if these are separate batches. I didn't do this with my latest interface because Meditech recommended that I not do it. I still wish that I had followed my instincts and done it as in the past with my other vendor.

We have Quest interface via NMI and I have set them up as non-numeric
because they could send results as "SEE NOTE" or "SEE BELOW" or "< OR =
0.90".
