This question is regarding the "Nursing Kardex". Remember the old paper
kardex??? Wondering what sites are using in meditech instead of a paper
Kardex? Have you developed an intervention that is similar to a kardex
or do have another process??

We use the patient profile for out Kardex. Information that they held previously on the paper Kardex they linked into the patient profile. We've customized one for the CRNA as well so they have the information only they need to see. The Kardex are scheduled to print 1/2hr prior to when the nurses arrive so that they can just pick up their assignments and go.

We have built our Kardexes in the Patient Profile routine in NUR. There is some flexibility and you can get creative with it. Our Kardexes are scheduled to print to the units once or twice a day and can be printed as needed. I have seen other hospitals say they have created NPR reports to replace kardexes.

We use Patient Profiles that automatically print at the beginning of
each shift.
We have several different formats: 3 views for Registered Nurses (only
one auto prints but they have the option of printing their
own if
they want a less complete view)
1 view for CNA
1 view for Nursing Students

The nursing staff was very reluctant to give up their paper kardex but
when we actually made the change it went smoothly.
The biggest complaint was that the print is small and the kardex format
is congested.
One thing that helped was we limited which interventions we send to the
kardex by using print number.

We use an NUR Profile that prints daily (and prn) to our Med Surg Area.
ICU still uses the old paper, hand written Kardex.

I created a very comprehensive Kardex using NUR Profile. I began with a
CDS from the assessments that creates a SBAR picture of the patient.
The diet and Nursing Care Orders (NCO) follow. These NCO are all of
those things that use to be written on the Kardex or in the margins.
Many are free text and give the Unit Secretary plenty of room to
communicate information. I tried to stick to one page, but the nurses
wanted more. It now includes all orders, lab results, meds, consults,
I&O grid, and specific interventions (eg: blood sugar frequencies)for
the last 12 hours and future 12 hours. Little things like the DOB (that
every nurse asks their patients) are included in the footer in large
print for easy viewing. I, also, included a place for the nurses to
mark the hour that meds are to be given, diets checked, and rounds
completed (every 2 hours)--on the front sheet. When this Kardex is not
used, nurses complain about not getting a thorough report. I think the
future is to "view" the data (we also have Visual Flowsheet), but the
nurses like to "write" to learn the information. The automated Kardex
allows for visual and interactive learners.

Amber,
We also use a Print profile that we print before each shift. The staff
use them for their "brains." We are not fully computerized in our
documentation but will be making that transition by first quarter of
next year. We will need to look at the need for continuing with this
function. I have a hunch it will be hard for the staff to let go of. We
use it when we give report.
