We block overrides in the OR, PAUC, and some other ancillary areas, but you always run the risk of you block overrides of the physician or the pharmacist entering the order, the nurse giving another dose, unless you link the order. But, then if you have already given the med and the nurse can't document against the override, will they really go back and document against the entered order, if it is linked?
Any way you look at it, this is handled very poorly by Meditech. We went to Faulkner 4-5 years ago to see POM, and when I saw that this was the process, I told the Meditech person doing the tour that this was not good, and they, with an arrogant attitude, said that they had no problem with this. Too bad that they weren't a real healthcare professional who could see the whole picture.

documenting a medication that is a stat. We have Pyxis and know that we
can set it so that the medication appears on the eMAR when the drug is
pulled on over-ride and the nurse can document it at that time. Can you
give me the gotcha's on that process that we should be considering? We
are a bit concerned about confusion over what drug has been given if
they pull of over-ride of 100mg of lasix to give a 40mg dose and then 40
mg is ordered bid. The possibility of giving the medication twice is
worrysome. Is there some sort of rule we can build for the generic
without the dose that states this drug has already been given within the
last so many hours? Also, what happens if they pull a drug on over-ride
and then return it. We tried that in test and it did not remove the
medication from the eMAR.

Pat - Overrides and how they are handled on the e-MAR and in POM are an example of Meditech design at its worse. The current situation is that you can have Meditech fill in the the following prompt in the pharmacy toobox parameters: # HRS MATCH UNV ORDERS WITH DM. Then if pharmacy or a physician enters an order for the override, it will link that order to the override order. Now, given that this is what we have to work with, it does have problems.
1. The typical scenario is that the nurse removes the medication on override (we have certain medications that are set to override in Pyxis), it creates an order on the e-MAR. The nurse would then scan the medication and document it. Now, what you run into is something like we did; for a while, we could only get Dilaudid 2 mg ampules. The nurse withdraws one ampule. They give 1 mg IV. Now, if you look at the order on the e-MAR, it says 2 mg for the dose and the route is IM because Pyxis pulls the first route it alphabetically sees out of the drug dictionary as IM instead of IV. You can change and should change the dose to 1 mg when scanning, but the only to put IV for the route is to require a site for Dilaudid. Now, the physician or pharmacists enters an order. Based on the pharmacy toolbox setting they will be prompted with a link query (and they should link) if within the time you set. So, you link it. Now, if you try to document against the order which the physician or ph
armacist has entered, it will tell you that it is a linked order and won't let you document. However, unless you have PHA DTS 11452, you CAN document against the physician or pharmacist order, something you definitely don't want to happen.
2. If you first do an override, then enter an order through pharmacy or POM, then document, the override order will turn to VIEW ONLY and won't let you document against it. This would be great if things happened this way, but they never do in real life (it will almost always be the first scenario).
I have been trying to get Meditech to have VIEW ONLY appear in the first scenario, but no luck so far.
As you can see, you'll want to do lots of testing and even if everything works, it is a bad design. There should only be one order (the override order) and the physician and pharmacist should be able to change the route, dose, etc on that order, rather than another order being created. Please let me know if you find any other problems as we had the link turned off up until now, but since we are going live with POM in the ER, it will get turned on tomorrow. We are Magic 5.62, so CS and later editions of Magic may differ from what I see.

For workflow we found it helpful to have the pharmacists use 2 logins
and view the emar while they are entering orders. We use the dispensing
machine overrides on the emar and have the nurse document on them. Our
nurses are more comfortable finding meds on their ADC than in the order
entry part of the emar (to populate the emar). To get rid of times off
the emar the pharmacist can do a fix dc ("FDC") on the order and move
the dc time to before the time on the emar. For dispensing machine
overrides/one time orders, etc. that d/c 1 minute after the start time
just make the d/c time the same as the start time and the time on the
emar disappears, I like to see the info in our ADC system that they
returned or wasted the med before I do this though. That or the nurse
has to document on the time as not given.

I want to default an ORM field (SCH.PAT.actual.primary.op) onto a CDS attached to a NUR intervention, so if the patient has had surgery, the surgery name will default in when they enter through the field (or when the screen is brought up). It seems it should be doable with some kind of DFT attribute, but I don't know where to begin.
Any ideas?

We are Magic 5.63. We answer the smoking questions in the general admission assessment. We have the smoking as a yes/no query that is required. If answered yes, another required query is if they wish to quit. A multiple query is used to list the types of smoking or smokeless (chewing tobacco) used. We also have an exposure to second hand smoke query. This hasn't been mandatory, and many weren't asking the question if the person was already a smoker; however, apparently it makes a difference if they are also exposed to second-hand smoke, so we will be making this query required as well. We have an NPR that pulls this information.

We have been pouring through the meaningful use criteria as well. And yes we will be changing our assessment to match how ARRA is requesting it be set up. Meditech is aware of the changes that need to be accommodated for meaningful use, hopefully they will incorporate a group response to go to the Past Medical History. I have not heard that they are going to do this at this time, but I haven't they aren't either. Currently as it is set up, we are going to be building it for just the requirement. We will also have the Hx of smoking questions so that they will go the PMH in the EMR, because if all we have is the group response and it does not going to the Hx panel with all the other queries then it will serve no purpose other than to fit the requirement.

I have a request from one of our pharmacists to write a rule to accomplish the following:
Currently all prescriptions are defaulted to FAX per the access dictionary. We would like to override that default with a rule to auto-print any prescription that has a control schedule defined in the RXM drug dictionary.
Does anyone out on the List serve have such a rule already written? If not, any help would Be greatly appreciated as I have never written a rule in RXM before and Meditech is not of much help with this issue.

Would you be able to tell me why you DON'T like your current thermal printer for pharmacy labels?
Does it not do bolding or sufficient fonts?
Does it not do tall-man lettering?
Does it not meet your productivity expectations? Possibly it is printing too slowly or jamming too often?
Are you dissatisfied with the manual of directions?
Other dislikes?
We will be going live with C/S 6.05 in 2011, including bedside medication verification (BMV), and would like to get a printer installed for our Application 1 training in November. We believe it should be a thermal printer and be capable of 2-D barcodes. I did review previous mtuser postings on printers as well as http://meditech.com/prbmv/pages/BMVciASpreliminaryhardware.htm and http://meditech.com/prpha/pages/PHAciAScsPrintDrvrsExt.htm for Meditech's suggestions, but certainly appreciate your insight.

I would like to build a table within an ITS.RES fragment report to count
the number of reports each doctor has in "Draft" status, so I can
reference the count in an MRI.ICR (main) report.
The department (ITS.RES.dept) is always "DPT.HIS".
Since I am only looking for report status of "Draft", the field
ITS.RES.rank.status will always have a value of "2", correct?
I am calling the ITS.RES fragment from the MRI.ICR main report using
footnote "AL START start".
For the ITS.RES fragment I am using the following detail and index file:
Detail DPM: ITS.RES Detail Segment: main
Index DPM: ITS.RES Index File:
status.x
I tried using a nested DO loop but was getting an error indicating the
status.date field was not found.
The main MRI.ICR report will have the following output:
Dr. John Doe has ## reports in Draft status.
Where ## is pulled from a computed field with a value of
/R.FRAG.VAL["DOE"]
Any help that I can get will be greatly appreciated.

I am in the camp of Keep It Simple if at all possible.
I do not know the configuration details with the Training ring.
Does the Training ring point to LIVE dictionaries so that keeping them
in synch becomes a non issue ?
Even if that would be the case, its just another option that the user
needs to remember to use.
And when your facility is in the midst of a Meditech update, that means
there could be a possibility of THREE options when signing on to the
Meditech system. Could be an extra level of confusion there.
Are there some advantages to a Training ring besides keeping the
dictionaries in synch ?
I would be very interested in these advantages and what reasons Meditech
would tout a training ring ?
Or do they discourage a Training ring ?
Seems more additional work for them as well when doing updates.
Is it a big revenue generator for Meditech ?
I am thinking probably not.

Could you please put out an L-list request inquiring about NPR reports
that pull the
following turn around times for the ER:
- Median time from when the patient arrives at the ED to their departure

discharged home
- Median time from when the patient arrives at the ED to the time that
the patient is
admitted to inpatient or observation status
We also need to know the minimum time and maximum time for each of
these.
This is needed for the MHA Keystone ER Data Collection tool so maybe a
request to the
Michigan Meditech sites would be a good idea too.

We are currently a Meditech C/S 5.64 site and we use Siemens Magic Web. We are looking to convert to Mckesson PACS next year. Just wondering how many sites use McKesson PACS and if your facility is putting "all images" in the system. When I say "all images", I mean Medical Imaging, wound Care pictures, endoscopies, etc. If you are putting all images into Mckesson, do you have separate interfaces to get the wound care and endoscopy pics into Meditech?

Version 564 brings the ability to define Recall on the query level.
The SAVE QUERY HISTORY field allows you to enter YES to define any query as recallable. So you can make the recall decision query by query and only let them recall those queries which truly do not change from shift to shift.

We are having the same issue. An option is to use a miscellaneous test with a query. The office can enter the test in the query. The lab orders the test. This can apply for other tests that are not built in Meditech.

Tony, I've been struggling with the same issue receiving an order from LSS through OE. We currently use an order set in OE to transmit the orders to Lab, but LSS can't interface the order set. I initially was unsuccessful, but got something working this morning. We're Magic 5.63 and this isn't an elegant solution, but it works. I haven't approached the issue yet of label generation and specimen labeling with the "real" specimen number.
I created a single test (OBPAN)with OB as the department (to keep separate from everything else). It is nonnumeric, orderable, NONREPORTABLE, no broadcast, with AutoResult of "Y", AutoResultAction of "V", and HoursToAutofile set to zero. On page 3 of the dictionary, Y is the only allowable result code, which triggers my OBPAN Order Group. Reflex rule is simple and can use location or whatever criteria you want, but I selected patient sex:

[f rflx not ok]};
The order flowed through for the single test into LAB. A few seconds after I received the specimen, the Order Group tests, including the Type & Screen ordered. Order Group is set to SpecType as SAME, OrderType is ADD. It will require re-entering the Receive Specimen routine to receive the new specimens, and labels could be printed at that time.
As I indicated, not an elegant solution, but it presents an option to have one initial LAB order trigger BBK orders. If you currently order/receive the individual tests, something could be configured as a panel with a nonreportable component test configured as above to trigger the BBK order. That would require a little more thought about what triggers the reflex rule.
If anyone else has a better solution, I'm all ears. I only received one reply to my post earlier this this week with essentially the same question.

We added the "Uncollected" query only to our micro orders (not all lab orders) . Each of our Micro Orders has a procedure screen. We placed an attribute on the CDS on the procedure that defaults Yes if the user entering the order is a nurse and no if the user entering the order is a physician.
The assumaption is that if the urse is entering the order - they have collected the specimen. If it is a physician then the order needs to go to nursing to "collect" before it goes to Lab.

specimens that never get collected, or get collected the next day (and thus
generate a date/time conflict due to the incorrect specimen number). If we
default it to "No", the nurses complain that it is too cumbersome to go back
and collect the specimen in the computer later. Any suggestions?

Is anyone out there using Varian (Aria) for outpatient oncology with a
Meditech interface? We are going with Varian due to the fact our parent
hospital (a non-Meditech facility) is using this, and the physicians
travel between the two facilities, and in order for them to have
consistency in practice, they want us to use the same ordering system. We
are being told that oncology physicians can enter their orders into
Varian and they will populate in Meditech for pharmacy & print labels.
Anyone having hiccups with this? Does the facility have to have POM for
this to work, or do the orders flow directly into pharmacy, bypassing POM?
We are currently Magic 5.63.

Version 564 brings the ability to define Recall on the query level.
The SAVE QUERY HISTORY field allows you to enter YES to define any query as
recallable. So you can make the recall decision query by query and only let

The analyzer has two components, a dipstick component and a flow
cytometer.
The dipstick component will run QC into MT. We have a barcode# assigned
to our dipstick UA QC and use that to run the dipstick tests into MT.
The UF component runs special beads for QC. The QC will not run
correctly if it is run as a patient specimen. The analyzer must know it
is QC in order to measure the beads correctly. Currently we have no way
to run this QC online. We run it manually and use a QC worksheet to
enter the results.
I'm not sure I would call one way better than the other but we do feel
your pain :-)

We are setting up a new interface for the AUWi (Atlas Urinalysis
Automated System) w/ the WAM (data manager/server).
The WAM will now allow for a specimen to have the same identifier and be
sent over the interface. Thus, we are unable to use the QC barcode
process for scanning as id#s for the qc specimens when we load them on
the instrument....
We have been given the solution to run our QC as patient numbers and
then unmatch them and rematch them to QC spec#s.....
Anyone else figured out a better way??

Are you interfaced direct to the Atlas (dipstick component) OR via the WAM??
We are using the WAM as our interface middleware. It links the two instruments together and it sends the data to Meditech. When we use our QC barcode#, the WAM does not like it because it as already seen that barcode# before....
If you are using the WAM, how are you getting it across to Meditech?

The analyzer has two components, a dipstick component and a flow cytometer.
The dipstick component will run QC into MT. We have a barcode# assigned to our dipstick UA QC and use that to run the dipstick tests into MT.
The UF component runs special beads for QC. The QC will not run correctly if it is run as a patient specimen. The analyzer must know it is QC in order to measure the beads correctly. Currently we have no way to run this QC online. We run it manually and use a QC worksheet to enter the results.
I'm not sure I would call one way better than the other but we do feel your pain :-)

We are setting up a new interface for the AUWi (Atlas Urinalysis Automated System) w/ the WAM (data manager/server).
The WAM will now allow for a specimen to have the same identifier and be sent over the interface. Thus, we are unable to use the QC barcode process for scanning as id#s for the qc specimens when we load them on the instrument....
We have been given the solution to run our QC as patient numbers and then unmatch them and rematch them to QC spec#s.....
Anyone else figured out a better way??

Is anyone struggling with Pharmacy POM rules like we are? Meditech has 258 rules out on the website but most are just repeats of the previous one and aren't really that helpful for me. We have issues with the queries being answered on the CDSs and the users have to click on the Req more than one time, among other things. My Meditech consultant is not too helpful as he hasn't much experience and when I asked if there was an advanced users guide I was told that several people have asked for it and it is "in the works"!
Any help would be appreciated. PHA rules that stay in Pharmacy are OK, its just these POM rules!!!

The way to figure out what your thermal printer can do:
1) Load a free trial copy of "Bartender" from seagullscientific.com
2) Load the seagull driver for your printer model
3) Design a label template and look to see what "Printer resident" fonts are available (the ones with the printer vs the "TT" (True Type) icon. If the font is printer resident, you could get the printer to produce it with a command string. If the font is "TT" (True Type) it would need to be sent as an image from the PC, which is not practical from Meditech.
If you want to get the most out of your thermal printers, you might consider Bartender Enterprise or Nicelabel which can function as print servers and merge data from Meditech into a label template which could include true type fonts, graphics, photos, etc. Bartender sells on a per-printer basis and Nicelabel sells on a per print server basis.
Please note that I have not worked with any hospitals using the products in this way, I only understand from the vendors that they have this functionality, and I use Bartender Enterprise on my local PC (including its ODBC connectivity feature).
Nicelabel is supposed to have a feature to automatically detect file creation, so it is probably easier to exploit from NPR reports than Bartender.

One of our issues was also making sure the correct tubes were drawn and labeled properly to assure correct distribution for testing when received in the Laboratory. The panels we have an issue with are ordered and collected in a remote clinic, so we didn't want to re-label specimens for instrument testing upon receipt in the Laboratory. We didn't want to have to place additional orders upon receipt in the Laboratory. A related complication is results flow back to the clinic system and need to be tied to the original order to display properly.

Tony, I've been struggling with the same issue receiving an order from LSS through OE. We currently use an order set in OE to transmit the orders to Lab, but LSS can't interface the order set. I initially was unsuccessful, but got something working this morning. We're Magic 5.63 and this isn't an elegant solution, but it works. I haven't approached the issue yet of label generation and specimen labeling with the "real" specimen number.
I created a single test (OBPAN)with OB as the department (to keep separate from everything else). It is nonnumeric, orderable, NONREPORTABLE, no broadcast, with AutoResult of "Y", AutoResultAction of "V", and HoursToAutofile set to zero. On page 3 of the dictionary, Y is the only allowable result code, which triggers my OBPAN Order Group. Reflex rule is simple and can use location or whatever criteria you want, but I selected patient sex:

The order flowed through for the single test into LAB. A few seconds after I received the specimen, the Order Group tests, including the Type & Screen ordered. Order Group is set to SpecType as SAME, OrderType is ADD. It will require re-entering the Receive Specimen routine to receive the new specimens, and labels could be printed at that time.
As I indicated, not an elegant solution, but it presents an option to have one initial LAB order trigger BBK orders. If you currently order/receive the individual tests, something could be configured as a panel with a nonreportable component test configured as above to trigger the BBK order. That would require a little more thought about what triggers the reflex rule.
If anyone else has a better solution, I'm all ears. I only received one reply to my post earlier this this week with essentially the same question.

We went to Iatrics and had them develop a NPR report for our legal
record. It includes some of the PHA Med Administration Report but
deletes the scan/not scanned as well as other columns. We also had them
include all documentation and reassessments. They did a wonderful job
(as always) at a very reasonable price.

We are having a problem with our urine drug screen billing. We have 9 drug tests that are in our urine drug panel and are available individually. The tests do not have the modifier 59 since they can be ordered individually but whenever the panel is ordered this is causing a problem. We have a few other tests like this but our claim scrubber catches those but it does not catch these.
Has anyone else had this problem or have any ideas how we can fix it?

We're using Datamax I Class and Zebra printers. Had to get a new G
Series Zebra to support 300dpi printing for Hospira PCA Pump Label
printing. You have to program all your labels, but the printers work.
We've uploaded a bunch of our Magic pharmacy labels with 2D Barcodes to
the Meditech NPR Report Repository, just search for 2D.....

We will be going live with C/S 6.05 in 2011, including bedside
medication verification (BMV), and would like to get a printer installed
for our Application 1 training in November. We believe it should be a
thermal printer and be capable of 2-D barcodes. I did review previous

Our Facility is going LIVE with Meditech 6.0 on 9/15/10. We will not, however, be implementing our Employee Prescription Pharmacy until 11/1/10. We originally wanted to implement Employee Rx, but we were unsure how to establish it. Meditech has not given us clear descriptions as to how we can establish this for Pharmacy considering the follow requirements:
1. Ability to add new Employees & Dependents when they bring in Rx's (limited Admitting function). This should be restricted to Employee Rx ONLY.
2. Ability to add/modify/delete insurance information for patients.
3. Ensure Employee Rx data is confidential from other departments in the organization.
4. Isolated inventory for Employee Rx.
5. Ensure charges do not flow over to the hospital's BAR (currently we just used a standalone cashdrawer, but is there a way to take care of this elsewhere in Meditech?)
6. Must be able to create a report once a month showing dispensing, amount paid & amount due from insurance.
8. Ability to add doctors into the system.
I was wondering how other facilities establish their Employee Pharmacies considering some of the above factors, keeping in mind we will also on 11/1/10 go to LIVE with a -FULL MMPHA- interface and eventually be moving to BMV by the end of the year.

I am trying to list a couple of NPR reports to .txt files to use as a
library.
Magic 5.61 does not allow listing reports to "DOWNLOAD" or "VIEW". I
can list a report to PREVIEW but this doesn't include any macros that
aren't called by that report. And, when filing a macro, the "Print On"
prompt won't allow PREVIEW.
My next option is to use option 36. to download the reports to a PC. I
seem to recall someone developed a "Viewer" for downloaded reports. Am I
dreaming?

The only downtime we have had in the last year was planned downtime for
either server replacement or Meditech upgrades. Longer term I am
planning to virtualize Meditech to minimize or eliminate downtime if I
need to replace a server. There will always be the possibility of
network problems.
About two years ago we had about 8 hours of downtime due to air
conditioning not being adequate to keep our server room at a decent
temperature. At 115 degrees we had several Meditech servers crash.
That was repaired temporarily be portable AC units and permanently by a
new AC system for the server room.
Also we now use IPeople Offline to make records available if there is
downtime.

For those of you who have trained nurses for TAR, what did you use as a
source of blood to scan the unit barcodes? I ask this as our inventory
is not large enough to do all training. Did you generate your own
barcodes? If so, did you use an npr report to generate the barcodes and
would you share?

We are a Meditech Magic / 5.63 version. Our ED department has been getting "Memory Buffer Errors" when trying to file orders on a patient. It doesn't happen all the time. Usually it happens on one patient and you have to enter thru all the errors and continue. Then sometimes it happens to several patients and locks the computer up causing the user to reboot their PC. We are working with Meditech and have loaded several DTS, but it's still occurring.
1. Has anyone else experienced this problem?
2. Have you been able to fix it, if so how?
