[MEDITECH-L] Wound documentation for multiple wounds

Hi All,
I am searching for information on how to create a CDS for documentation
of multiple wounds. What I was thinking was creating page 1 with the
wound assessment then having a way to prompt page 2 to come up if there
is a second wound and more pages if other more wounds. Wanting to know
how others are doing this.

I have a query on the first page of the wound assessment that asks "number of wounds". I have a DFT attribute on it to default the number that has previously been documented. Each wound is on it's own page. On page two, I have the following attribute on the queries:

IFE=[ANS,"NURWOUND16"]|0>"1"

NURWOUND16 is the query that asks the number of wounds. On page 3 where wound # 3 is, I change the 1 in the attribute above to 2, and so on.

We have our wound documentation incorporated into our shift assessment
(which can be documented multiple times in a shift). I've set up screens
for separate wound sites that are identical, with a Y/N query at the
bottom of each screen that reads: "Document additional site?" There are
then attributes on the subsequent screens that allow or prevent access
to the additional fields based on the response.

We have several interventions:
Pressure Ulcer, Assess Site 1-3
Pressure Ulcer, Assess Site 4-6
Pressure Ulcer, Assess Site 7-9
Pressure Ulcer, Assess Site 10-12
Wound (non pressure ulcer, Assess Sites 1-4
Wound (non pressure ulcer, Assess Sites 5-8
Wound (non pressure ulcer, Assess Sites 9-12

Each site has unique queries and the Nurse files when she had documented
the appropriate # of sites.
