Hi all,
We are a CS 5.5 using cost accounting and I need some help with some of
the reports. We came from Siemens Cost Accounting and they had some very
good reports showing direct, indirect, fixed, variable information that I
have yet to find from Meditech. I need a report that shows where all the
indirect costs are allocated and if they are fixed or variable. We have an
NPR that shows us all the cost groups by cost center after allocation, but I
need one before allocation. If any one uses cost accounting and has run
across any useful reports that can get this information, I would be very
appreciative. Thanks

Word Wrap field? We have some standard information to place in a CDS within
a Multi-line Word Wrap field during our triage assessment that we would like

document all users who have accessed a specific patient's record. MT has
proposed setting up a complicated mechanism which directs log files into
a directory on our HUB server. From there, the data would have to be
manually ported into an Access database. It's not a solution that's
going to work for us.
Viewing what a single user has done is no problem, but that's not what
we need, either.
Has any one accomplished this via NPR?

We are Client Server 5.54, Pyxis Medstation 3500.
When a user enters an Rx order in Meditech with a stop date and time, the
original rx message sent to Pyxis has the stop date and time in it. Pyxis
is responsible for adding the stop date and time to the rx order in the
Pyxis console. You probably verified this when you did your integration
testing. Pyxis has 2 settings on the device, order lead and order lag (or
something like that). This order lag will hold the order for the specified
time past the order stop date and time, defined in hours. I would check
these settings, if you haven't already.
If the order is D/C'd manually within Meditech, using the
modify/discontinue, you should see an outbox message going to Pyxis to D/C
it now.

I think Maureen's problem and what I see in McKesson is different in that
the RX is never DC'd on the Dispensing Machine if it is an RX with a stop
date/time entered in Meditech If a user manually DC's an RX or the patient
is Discharged then the RX is stopped but if it just reaches the stop
date/time Meditech does not generate an NMI message.
Your description of the PHA parameter is correct, but the reverse situation,
as this is were Meditech does send the DC message but the parameter delays
it for X hours to allow the RN to give more doses.
I have actually found it best to keep this at ZERO for most RXs but then
work with the dispensing machine vendor to delay the DC for RXs with a
schedule of STAT or ONE only to allow time to dispense the med before it
DCs.

there is a setting for a hold time for DC'd orders. Prior to our 5.61
Magic upgrade, the field was in days, and ours was defaulted to 7 days
so a discontinued RX transaction was not transmitted for 7 days.
MediTech could not understand why I would want this changed. I had them
change the time to 0 days and used the Pyxis setting to hold
thediscontinue for a couple of hours. With 5.61, the setting for days
was changed to hours it shows up in the toolbox as "DISP.M.INACT Rx
DELAY HRS". I have mine set to "4" so discontinued orders hang arround
for 4 hours. Hope this helps.

Pharmacist manually discontinues the medication in PHA. A Discontinue

entered in PHA, as indicated on the Order Detail History as Discontinued
by Scheduler.
Meditech has stated that "when orders are scheduled to be
discontinued, a DC message is not sent at that time. It is only when a
DC happens that isn't scheduled that PHA will send a message."
This does not make any sense to me as there are serious patient
safety/risk and pharmacist workload issues associated with this problem!
Has anyone else encountered this? If so, what was the solution? Do you
have a custom?

is what you were looking for.
DM Inact Rx Delay Days Parameter
Define the number of days that inactive medication orders (that is, orders
that are discontinued or cancelled) remain in the dispensing machine
interface index file.
When you define a number of days, you allow users time to enter debit or
credit transactions for orders before the Pharmacy Module deletes them from
the system. After the system deletes the inactive orders, Pharmacy sends a
deleted order message to the dispensing machine for medication orders that
were
? entered and then discontinued or cancelled in the Pharmacy Module
? created by interface override through the dispensing machine and
then cancelled

Thanks for taking the time to reply. We were considering LSS with the
full MPM suite, but we wouldn't be deploying a pretty "windows style"
GUI. Also, the functionality of the system seemed less attractive than
that offered for much less from other vendors, but much more integrated
with Meditech (of course).
We have learned that it is possible to use EDM's template style
documentation through PRV, but it involves adding another facility.
There is a clinic in Texas who is currently LIVE with this option.

We are looking for a way to enter SOAP note based clinical
documentation for our providers as well as basic CDS's for MD's and
the nurses. We have attached a couple of CDS's to the via Process
Encounters By Practice, where the user highlights the patient
and selects "Edit" from the verb strip, hits F12 and either selects
a draft status CDS to edit, or "A" to Add a CDS, and hits F9 lookup
to select a CDS. We also saw that there is a way to access Process
Interventions via the Practice Management Screen.
Here are some of our questions:
Did your physicians/providers document via PRV? If so, how?
(How was the doc attached/accessed and what format was it?)
No, they didn?t.
Did your nurses document VS via PRV?
No.
Is there a way to access and set up canned text notes in PRV?
I believe that there is.
Is there a way to default Today's VS to the physician's documentation?
Not sure.
Is it possible to integrate voice recognition software with PRV???
Not sure.
How did you print the documentation if a copy of the record is needed?
I think that there are some print routines in there somewhere.
And finally, our MIS Nursing Support Specialist would like to pull
PCM templates to PRV for physician documenation. Do you know
of a way to pull from another DPM to PRV?
No, I don?t.
Of course, we still have all the reminders/calls and scripts to
consider on top of the workflow considerations...

We have PRV and use very little of it. I am actually in the process of
getting our provider office online with Calls/Reminders with the
ultimate goal being to use the RX and referral portion too.
We used the product 5 years ago but sold the practice we had at that
time. Our current practice uses SCH and PCI but nothing else...

Hi Maureen,
This is actually not a bad thing. When pharmacy manually
discontinues a medication, you want Pyxis to discontinue the medication.
If you have NMI, then you will see DC in the message stream. Now suppose
you have an order such as cefazolin 1 gm IV q8 hrs x 3 doses. On the 3rd
dose, 1 minute after the medication is "officially" discontinued,
previously Meditech would send a DC message with the scheduler or I
think it was CA, and then the order was gone. 15 minutes later the nurse
went to Pyxis to get the last dose of the med. It's not there!! Pharmacy
had to then manually extend the stop date and time to cover for this.
Not a good thing. Now Meditech sends XO or change order messages for any
order "changes".
The way to handle this is on the Pyxis side. There are lead and lag
parameters on every device. Simply change those to a reasonable amount
of time you expect that a med should be active on the system before
dropping off. My lead time is 4 hours and the lag time is 6 hours. We
have eMAR/BMV and wanted to keep NOW orders on at least 6 hours before
dropping off from Pyxis.
If you are encountering problems with overrides, make sure the Auto
DC is set to Y on the Dispensing Machine dictionary if you have NMI.
I am very pleased with my current configuration. Let me know if you
have any further questions.

We went Live with two profiled Pyxis medstations and have encountered a
serious problem. Medications that are discontinued in the Pharmacy
module based on the Rx Stop Date & Time are not being discontinued on
the medstation.
After searching hundreds of NMI messages, I found that a NMI
Discontinue message is only being sent when a patient is discharged or a
Pharmacist manually discontinues the medication in PHA. A Discontinue
message is not being sent based on the medication stop date & time
entered in PHA, as indicated on the Order Detail History as Discontinued
by Scheduler.
Meditech has stated that "when orders are scheduled to be
discontinued, a DC message is not sent at that time. It is only when a
DC happens that isn't scheduled that PHA will send a message."
This does not make any sense to me as there are serious patient
safety/risk and pharmacist workload issues associated with this problem!
Has anyone else encountered this? If so, what was the solution? Do you
have a custom?

alerts (controlled w/ limited access based on employee's responsibilities)
