Hello all,
We are having a problem with our hospitalists knowing when the Medication Reconciliation is completed. I have talked to Meditech about it, the specialist says that there is an enhancement that we can choose to not allow “Continue from Ambulatory” until the medications have be reviewed. The problem with this is that we are having our ER nurses do a review initially then the admitting nurse does another review and adds the “Last Taken”. Does anyone have any suggestions on a way to notify the admitting physician the Med Rec is complete and ready for his review?

Have you thought about using provider messaging? When the admitting nurse has completed her review, she could send a message to the provider stating that Med Rec was complete and ready for his/her review. The message goes to the provider workload and you can set a priority to the message.

I actually presented that idea to the Nurse Managers last Friday after some discussion with one of the providers. The NMs do not want to add any more duties to the nurse so they were not very receptive. We are using the messaging with Pharmacy and it has been very helpful. I think it would be a good option too. Right now, I have told the physicians to verify that the Med Rec is completed when they are doing the rounds with the nurse (they have a 12 hour window to finish it). Not a good solution but not sure how else to do it.
At our facility, Pharmacy techs interview the patients using the External Meds information from Dr. First. Pharmacists enter the Medication History answering a query on the Pharmacy Patient Data screen that the Medication Reconciliation is ready for review. The response is pulled onto the nursing status board and the physician desktop.

We don’t have enough Pharmacy staff to be able to do this unfortunately. We are not using Dr First yet, plan to later this year though. Sounds like the Provider Messaging is the best bet….. even though they are not that interested in it. Thanks for the information!

Our sites have been using a query that is set to Complete once the Home Meds have been reconciled and we then share this on the Provider’s Rounding List.

This is a good idea! How do you have the query set to complete? I’m not sure how to do that piece of it. I know how to add to the rounding list after it is answered.

Hi All,
Our sites are able to update the Provider regarding the status of Home Medication Reconciliation. From Nursing documentation (MDRO/Initial Screening), we have a launch to Med Rec. Once the nurse completes this they are then brought back to the documentation (MDRO/Initial Screening) where they are required to answer the Home Med Status query prior to filing. This then is available in the CCDQR to be pulled to the Rounding List. No need to share the query!
