Hello All!!!
I am new to this list, and I hope that this issue has not been covered already. I have noticed a few posts concerning UB04 printing, and also settings that have worked for other organizations. I have also seen the Meditech Knowledge Base articles on this. I have run into a problem that I believe may be a problem across the spectrum for people who are using HP LaserJet 4250’s to print their UB04’s.
I have been able to configure our 4250’s to print “correctly” using slightly different settings that Meditech recommends. Nevertheless, I ran into a problem that either Meditech is unaware of, or one that should prompt them to update their online documentation. According to our billing specialist, everybody will soon be required to use a 4-digit code for Type Of Bill (the field at the top-right of the UB04L form). Currently, Meditech on our system is configured to use a 3-digit field, and these claims are printing without any problem. However, when we use a 4-digit field for this field, the 4th character gets truncated. After a few hours of messing with settings, along with a call to HP Technical Support, I am thoroughly convinced that this is a limitation on the “standard” HP LaserJet 4250 printers. I was told by HP that in order to use the “edge-to-edge” printing feature, we would have to purchase an additional proprietary compact flash card from HP at the cost of $750 a card (not sure if that was MSRP or true retail). There is no mention in Meditech’s knowledge base that this specific option is REQUIRED by Meditech to get these things to print correctly.
My question to the outside world is this: Are your claims printing correctly using a 4-digit field for Type of Bill? If so, do you have the compact flash card option from HP?
Any input or discussion would be greatly appreciated, and I would be glad to be more specific on my explanations. I couldn’t even find the “Edge-to-Edge Override” option on any of the 4 BRAND NEW printers we just purchased, and the tech from HP said that was not an option on her printer either. If this compact flash option is REQUIRED, it would AT LEAST be nice if that was mentioned in Meditech’s documentation so we could correctly estimate the total cost of moving to an edge-to-edge printer. $3000 is a lot of money to be off by in my opinion…

Unfortunately, this is not a problem with Meditech at all, because if it was, I’m confident they would have a resolution. The edge-to-edge override option is key and this is not available on the HP 4250. My recommendation is to save yourself many more headaches and purchase the HP9050n. A bit more costly, but worth the money. Following the purchase of 4 of these, we no longer have issues with printing the UB04.

Well, I have some good news (and hopefully my last post on the topic).
After many tedious and excruiating hours of testing, I think I have found the settings that will work. The keys to getting this to work are:
1. First, finding the CPI settings that will allow ALL characters on the line to print.
2. Then, finding the proper horizontal edge-to-edge setting that aligns the form correctly.
By no means is the form perfect. Unlike Meditech's programming for the UB92, which they did a very good job on and is almost flawless, Meditech's programming of the UB04 has left a lot to be desired; in fact, I would be embarrassed to send this form out to payers with the way that some of the fields are lined-up within the blocks. But it DOES print all of the characters, and for the most part, the characters that don't fit nicely within the blocks are probably not critical data fields.
Here are the settings that I have found work the best:
ELF:
CPI: 10.12 (this is the key; it is the smallest value that allows all characters on the line to print)
Left Margin: 0.20
LPI: 6
Top Margin: 0.095
HP 4250 Printer Quality:
X1: -4
Y: 0
Good luck, everyone.
