In regards to the critical test result; we have a mini assessment for
critical values reporting. The nurse documents the time, date, result,
verification MD has received the critical result, and an action (orders
received, etc.).

Wondering how others are capturing the following data-

In Michigan when a HIV test is ordered, nursing must document education
was provided. Wondering if anyone has a creative way to have this
populate the intervention list so it is not forgotten?

Also, wondering how others are handling the nursing documentation of
critical test results when they call it to the physician?

We demo'd several different brands for BMV last year and went with the

We over here at Bob Wilson are in the process of vetting computer carts
and are leaning toward Metro flo carts particularly the 1760 model. They
offer all of the attachments you have listed as well as others, as do
any other the other cart manufactures we have looked at.

I like the Ergotron carts as well. However I prefer the Stinger Medical
carts. I have used their products for 15+ years and not only is the
product excellent it is durable and their customer support and
maintenance cannot be beat. I know places still using some of the carts
they purchased 15 years ago!

I am sure most of the vendors will be at MUSE the end of May.

We're partial to the Rubbermaid carts from the IT perspective. We've had
some Ergotron carts from 3 years ago that were quite bulky. We also have
some Flo carts but the computer hardware has not aged well and cannot be
replaced without replacing the entire cart. With the Rubbermaid carts we
can place small form factor PC's (we use Dell Optiplex 160's) in the
lockable computer part, but then you can have drawers below as well. No
exposed computer hardware (except for the monitor), and can be
upgradeable when the computer becomes obsolete.

Is anyone out there in L-land using scribes for order entry? While the
Joint Commission does not endorse the use of scribes, they have stated
that if the organization decides to use scribes then all entries must be
signed by the scribe along with title (role), date and time. The
physician MUST authenticate all entries by signing, dating, and timing,
and orders CANNOT be acted upon until authenticated by the physician
working with the scribe. OK, so how do you meet that requirement? We
looked at having the MD log into his POM session, the scribe would enter
the orders, and then the MD would review and enter his PIN, but there is
not documentation of the scribe ever having been used. We also looked
at setting up the scribe as a provider that could enter orders but
require a co-signature, but then once the scribe enters the order -
those orders are automatically sent to the various departments to be
acted upon BEFORE the physician authenticates by co-signing the order.
Any
body have any suggestions???

The integration within MEDITECH's EDM product is critical in deciding on

MEDITECH vs. Other Vendor. We have been LIVE with MEDITECH's EDM product
for
over 2 years and if anything, wait time has improved. This is not due to
the EDM
implementatin alone, we've also developed ED Task forces in order to
determine
areas where slowdown was occurring, etc. When we initially went LIVE?the
turn
around times in our ED were greatly increased (which we anticipated and
were
open and honest about up front) until people got up to speed. The trade
off
was?was well worth it as far as the immediate availability and accuracy
of ED
documentation is concerned. One of the keys to our success in the ED was

having?administration on board, a clear project plan, and people running
the
project that were fully invested in making it a success. We also spent a
lot of
time identifying specific workflows in the ED and ensuring that we could
provide
an electronic equivelant when appropriate. I also agree with others in
making
sure that you really investigate your options thoroughly as far as using
any
other vendor system. I won't elaborate, but I can speak from personal
experience
that the integration can be a nightmare no matter how pretty the product
looks.?

We are using BMV in our ED, and have MEDITECH's EDM product.?For us, it
really
seemed to haveminimal impact on nursing. It actually helped with getting
the
physicians to enter 100% of their orders via CPOE since we did not want
nursing
doing overrides on our Pyxis machines in order to retrieve medications.
It has
impoved the safety of medication ordering and administration in our ED.

We recently went LIVE with Reconciling Medications through RXM. When
the patient is admitted to the hospital we are updating their medication
list & entering the date/time the meds were last taken. If the patient
has a procedure done & it is determined that he/she needs transferred to
another facility, it would be nice to be able to print the meds that
were entered as well as the date/time last taken. I'm able to get the
meds to print on a NPR report written out of RXM but I'm not able to get
the date/time last taken to print. (Note - the date/time last taken is
the info entered while reconciling the meds, not the date/time the
patient received the med while hospitalized.) Has anyone been able to
get this information to print along with the meds listed?

Any help would be greatly appreciated!

We have been live with the TAR since July and have not seen this issue.
We are 5.64PP21. The biggest issue we see is the that when the staff do
the verify and have their co signer do their user name and pin....it
doesn't get filed. Then they can't do anything. The other issue we see,
but not as frequently is that our labs cutoff time for units is 6 hours.
The lab auto completes. If staff have not done their documentation
(because what is "Point of Care").....then they can't document on the
blood.

I'm not sure if we have had a pt with all of the BBK products ordered
but I have seen both RBCs and Platelets ordered on the same specimen
without problems in TAR. I did have an email conversation with our BBK
director and she has not heard of it either.

rbc's), all of the documentation in the TAR, including vitals signs etc
go across to LAB BBK as "comments" and the large "size" of the comments
cause it to hit the fail queue because they are too large to process
through.


Is this happening to anyone else?

MT is working on a DTS to resolve, but in the meantime have suggested
"splitting" the specimen to decrease the size by spreading it across
specimens, which we do not think is appropriate to do.

Any ideas?

I think Meditech's suggestion to split the products between various
specimens is ridiculous!
Part of having a safe transfusion service is maintaining specimen
collection information and ID. Introducing new requisitions for the
same specimen is crazy.

Can you tell us how many products it takes to cause this 'melt-down'?
8-10..., 20-30...? 50+?
And is it just the number of products or a variety of products?

What happens when the HUB queue fails?

(Is this Magic or Client Server?)

We are not using TAR yet, but are trying to work in that direction.

I echo both statements below. EDM has been running for years without any
issues and the ED staff love it.
I was not part of the original project so cannot comment on
implementation issues/concerns/lessons learned.
We have approx. 200,000 ED visits per year over 8 sites; 3 sites have
large ED's and the rest have smaller urgent care centres.

I would have to agree with Jim's comments. EDM works very well in
hospitals seeing less than 30-40,000 patients a year. I have seen some
issues in viewing when you get upwards of 50K a year but not
insurmountable.

Physician input is key I think.

TAT is equal to a manual process and I did time studies on that 2-3
years ago. Now if you are bringing on BMV and physician OE in the ED I
believe you could see some increase in TAT but they should decrease
rapidly once all users are comfortable with the system.

We recently have decided to ask ALL of our providers to electronically
sign their dictated reports in
MEDITECH . Up until now we didnt push the issue and let them do it or
not. Now with moving to CPOE and sending reports to
Other systems we are going to require it .

Wondering how other sites are handling this issue ?

If we have two providers/collaborators on a ITS report , if one provider
signs ,the status goes to Isigned . Once the other provider signs , the
report goes to Signed. The draft report goes to both of their signature
ques so this is not a problem. This would occur for example when a PA
and a MD are working together on a patient. The PA dictates the report
and both view and sign.

The issue we are having , is if an addendum is added by the PA , the
report only goes into the PA's signature que . It does not go into the
MDs signature que. The same would be true if it was another MD on the
report. The only que the addendum report shows up in , is for the
provider who dictated it.

Our medical records dept feels it should go to both ques. We asked
MEDITECH about this and their stance is that was by design .

How have other sites handled this , or is it a problem for other sites ?

stores the time as a Meditech timestamp. This can be converted to a
normal
SQL datetime datatype using a simple function. Create this function in
whichever database you already store your reporting code (stored

We are Magic 5.64. I have a question about term server settings. We
are having to set up a new (not purchased new:-)) term server that is
emulating our old xyplex term server. I guess I was spoiled because in
previous set up all we had to do was make sure that the baud rate, stop
bits, data bits, parity were the same on the term server and the
analyzer. I knew there were other settings but they must have been set
correctly out of the box. There are many facets to my issue but suffice
it to say, I do not have access to the exact settings from my old set up
and this new server had some default settings that I had not seen
before. My question is how do I know what settings need to be
enabled??????? How can I find out what all these options are even for?
As you can see I have very limited knowledge on this issue, and ANY
assistance is greatly appreciated.

We're partial to the Rubbermaid carts from the IT perspective. We've had
some Ergotron carts from 3 years ago that were quite bulky. We also have
some Flo carts but the computer hardware has not aged well and cannot be
replaced without replacing the entire cart. With the Rubbermaid carts we
can place small form factor PC's (we use Dell Optiplex 160's) in the
lockable computer part, but then you can have drawers below as well. No
exposed computer hardware (except for the monitor), and can be
upgradeable when the computer becomes obsolete.

We over here at Bob Wilson are in the process of vetting computer carts
and are leaning toward Metro flo carts particularly the 1760 model. They

I like the Ergotron carts as well. However I prefer the Stinger Medical
carts. I have used their products for 15+ years and not only is the
product excellent it is durable and their customer support and
maintenance cannot be beat. I know places still using some of the carts
they purchased 15 years ago!

Hello!
We are in the process of deploying 10 Star micronics TSP800 thermal
printers for prescription printing and I have run into a problem.

We are a CS 5.64 site and we have two Star Micronics TSP800 working in
ER. They are using the older 05/06 LAN card. When we ordered the 10
additional printers, they came with the new 07/08 LAN card and we have
not been able to get them to stay online.

When I look on the background clients, the printers are all saying Ready
but when I try to print from my desktop, it says offline and the
prescription is held in the queue. Then, after several minutes
(sometimes over night), it prints and goes offline again.

Since the two in ER are working, we thought that it might have something
to do with the new LAN card version so Star sent us the older 05/06 to
try. I installed it and it is still staying off line. Has anyone else
run into this problem?
Any help is appreciated!!

Can you clarify if the VPN solution use are using is managed by your
hospital or is it provided (and managed) by Meditech/Dell Meditech
Solutions Group/Park Place?

We are looking into virtualizing Meditech into a VMWare environment and
this is one of the stumbling blocks. I haven't spoken to Meditech yet,
but I have been told repeatedly that Meditech requires us to install and
pay for their VPN solution for remote support.

I further understand that we are using the same equipment and software
that Meditech uses for their VPN. Can you elaborate on what solution
you are using?

The best advice our sit visit told us was NOT TO PRINT THE ORDERS...

One example they related was the athe staff would not act on the orders
until the sheet was in the chart!!!

We have a profile (NUR) that runs every so often that is taken to a file
that is pushed out to our downtime PCs where the labs and eMARs are..
Many are using other vendors for a downtime solution..

We never printed.. And made sure our orders are archived with the audit
trails.

Beware.. Once you start is it VERY HARD to take back..

We have both the Meditech VPN supported by Dell, that is as it was
explained to me a requirement of use of Meditech. This is a Cisco router
with specific programming to allow the secure access from Meditech
facilities as well as LSS (now Meditech). On our side of things for user
management and remote support (so as to not have to show up on site
every time a password needs reset) we use a Sonicwall SSL-VPN to connect
to the system and remote into a PC on the network. The SSL-VPN ties into
Active Directory from our domain so that users of the remote access do
not have to remember yet another password. For our Remote
transcriptionist we have her use a more traditional VPN connector though
the same SSL-VPN box so that she may have "local" access to resources
that she needs.

Can you clarify if the VPN solution use are using is managed by your

We are looking into virtualizing Meditech into a VMWare environment and
this is one of the stumbling blocks. I haven't spoken to Meditech yet,
but I have been told repeatedly that Meditech requires us to install and
pay for their VPN solution for remote support.

I further understand that we are using the same equipment and software
that Meditech uses for their VPN. Can you elaborate on what solution
you are using?

Thank you so much for responding to my message. I wasn't sure it went
through because Shirley Shae could not see it. My question is did your
Hospital purchase SSI's ClearView (test database for 5010.. If you
didn't how are you testing. Per my SSI person they will not load the
5010 edits in ClickOn until the payer(s) go live with 5010's. Which
means we can't test them. We can send 5010's to Clickon but they will be
billed to the payers as 4010's until 1/1/12. I am very interested in how
your hospital is handling this.

Hello everyone,
For anyone who is on LIVE with PCM1, did you enable eSignature? If so,
how did you deal with the physicians signing both electronically as well
as in paper. For example, if the physician gives a telephone order, they
will be given a Notice of unsigned orders on their notice list. They
will then go ahead and sign electronically. But since we are not live
with CPOE, we are still utilizing paper chart to sign ALL orders. Won't
that be double work for the doctor's because they would have to sign
electronically and the "paper" order that the nurse wrote for telephone
orders?
Did it get too confusing for your HIM dept. to figure out which
physician's are eSigning and which are not?

Roger,

It sounds like you need to make a lab toolbox parameter change. It
appears
that the parameter for holding this data is only set for 45 days. You
will
probably need to extend this out to a year. MEDITECH will have to make
this
change through a task. Please be aware that if you make this change,
you
will not automatically be able to look back 365 days immediately as
MEDITECH
is cutting off anything older than 45 days. Once the parameter is
changed,
you will then be able to look back 46 days, and then 47 days.... as each
day
you will accumulate more data.

Subject: [MT-L] Magic 5.62 - Blood Bank

I am writing a report to capture all the patients who received FFP along
with the date given. I have a report that works using the
lab.b.spec.transfused.index. Unfortunately, that index only holds about
45 days worth of data. I tried running the report with no index, but I
only
get a sum of all the units administered without being able to separate
them
by administration date.

Is it possible to get a listing of these patients going back farther in
time
and sort by date?

Thank you so much for responding to my message. I wasn't sure it went
through because Shirley Shae could not see it. My question is did your
Hospital purchase SSI's ClearView (test database for 5010.. If you
didn't how are you testing. Per my SSI person they will not load the
5010 edits in ClickOn until the payer(s) go live with 5010's. Which
means we can't test them. We can send 5010's to Clickon but they will be
billed to the payers as 4010's until 1/1/12. I am very interested in how
your hospital is handling this.

In Magic - we are able to print a field sideways on a piece of paper.
We used it for one physician office who liked the patient name to appear
in the gap on their manila folders and did not have to do labels. Since
we have converted to 6.0, I can't make that work through the NPR report
writer. Does anyone know if this is even possible in C/S?

We do the same thing but have found that this is very confusing for the
Provider Covering functionality - Which doctor Jones do they add to
their Covering List? Our work around for now is to cover for a group
rather than an individual.

We create separate provider mnemonics for the different locations they
order from .
For example ...

DOC1 - original provider mnemnonic
DOC1@CLIN1 - New mnemnoic for same doc at different location DOC1@PSY -
New menmoinc for same doc at psych location

It is cumbersome and confusing for the users. Not to mention if doctors
need to sign reports for each of these mnemnoics You have to have a
separate username for each and use the Alternate Signing routine to
allow the same person To sign them all . Crazy.

Somewhere out there, someone must have figured this out.

A provider who works at several locations, orders a study. The patient
presents, the service is provided, the report needs to be sent to the
correct provider's location where the patient is receiving care.

We are looking for the opportunity for the provider field to be a drop
down which allows the user to select which location the provider was
practicing from - thereby will drive the final report to the correct
destination.

How have others handled this?

Question for hospitals that turned on Order Source function throughout
hospital.

We turned on OE Order source and noted that the order source does not
cross over to LAB. Meditech is stating it is working as designed.
However, it does default into $RAD module.

What are hospitals doing for LAB order source.. Are you Defaulting in a
Order source for them or making them manually pick the order source when
in E/E Req. Note if you are using AOM in the offices no order source
defaults in either.

Is it possible to control what database the NPR report pulls based on
where the user is when they run it?
For example, I have 5 BAR databases, and I want one report for all the
facilities. Can I code so that user does not have to pick BAR.XXX when
they run it - the NPR report knows to use it? Along with this, I know
that you could code a frag to look to the what ever database is being
used..but I can not remember that coding. Could you tell me what it is?

I do not think this is possible in C/S. The Print Manager seems to
block certain PCL-5 commands, but allows others to get to the printer.
Print Direction is one of the blocked commands. Print position, and
push/pop printer cursor do get through.

You can toggle "pass-through mode" with a command, but we have found
that if you do this, all the graphics on the report are ruined. The
command to toggle pass-through is .^!T Meditech internal documentation
says "This mode can only be changed on page boundaries".

We have struggled to see if there is a way to send PCL-5 commands that
are otherwise blocked by the print manager, but so far we have not been
successful in a way that preserves the graphics and formatting of the
report.

We are moving to CPOE and provider documentation the end of June. We are
looking for a solution to messaging and how to get messages from nursing
and other disciplines to physicians and back to nursing. I have set up
the messaging system in PCM but have found that there are limitations to
using this system. I am looking for other sites that have found a
solution to messaging. Thank you for your time.

I am looking for a procedure to launch the EMR for our outside physician
staff. I know of one in LAB, but I think I have seen one for HUB or
maybe a Z.program. You know how that goes; when you need something, you
can't find it!

Make sure the Earning codes and Employee #s from Meditech are the same
in Kronos. It's a 4-5 field NPR report that you upload every pay
period. Kronos should be able to help with the upload setup.
