Make sure all forms meet MEDITECH SCA specifications, MEDITECH has them
posted on the Product/Resources Homepage. You also want to pay special
attention to margins, barcode type and size, and space around the
barcode.

Subject: [MEDITECH-L] Form Fast and SCA
We are about to start building forms and/or consents with a company
FormFast. We plan on going LIVE with Meditech's scanning and archiving
application (SCA) at a later date. Does anyone have any suggestions on
how
to implement FormFast so that it will correctly flow with SCA? How
should
we name the forms now correctly so they can be used in Meditech's form
dictionary when we go LIVE with SCA? Are there any restrictions to the
naming of the forms? How many characters are allowed? Can you use
numbers
and/or letters? Are spaces allowed? Any information would be greatly
welcomed and appreciated. Thanks, Barbara

We use FA and SCA here. You can have up to 15 characters for your form
names in SCA, but the majority of ours are under 10. You can use spaces
and symbols too if needed. We use alpha names for most of our forms,
but do have a few that are numeric and alpha-numeric. We do not have
any with spaces or symbols, but you are allowed to do so if you wish.
We followed the recommendations from the product and resources home page
when defining our forms; specifically the barcodes and forms
considerations documents. http://www.meditech.com/prsca/homepage.asp
We use 3 of 9 barcoding and it works well for us. We also had existing
standardized forms that made it easier for us to barcode for this
project. We put the form barcode in the lower left side of our form
where the form description is and the patient barcode is printed or the
patient label is attached in the lower right side of the form.
Hope this helps.
