[MEDITECH-L] dietary orders

We are having an issue (this is not new)with dietary orders. Here is a
scenario of what happens

A patient comes in and a diet is ordered in order entry. Later, the
doctor orders a supplement or a dietary consult.

We enter these in the same manner we order diets, Category Diet -
procedure- Ensure or consult. When this second order is placed, it
cancels out the original dietary order. The user is having to remember
to type in the original diet order

in a comment field we have on a CDS attached to the order. Does anyone
else have this problem or have found a solution to this problem? Any
and all responses are much appreciated.

We always order the primary diet as the procedure and enter on the
category screen the diet modifications. FORCE/ENCOURAGE FLUIDS,
FLUID RESTRICTION, NOURISHMENTS. Free text is also permitted.

Under the Dietary Procedures we have a regular and a special diet. The
special diet has a cds that lists all the different options - low fat,
low sodium, etc that they can choose.

Supplements are under the regular procedures, Category of DSN - Dietary
Special Needs.

There is a parameter in OE, under the queries subheading, that is for
concurrent Diet and Primary Diet. You create a CDS to attach to the diet
orders (in the dietary procedure dictionary) with these two queries..One
that asks: Do you want to keep the previous diet orders and the other
that asks: do you want to cancel all previous diets. These are Y/N
queries and depending on the answer is how previous diets/nourishments
are kept. That will solve your previous diets being cancelled when
entering new diet orders, (or cancelling them when you do not want to
keep them). However we enter our nourishments from a category called DSN
in our OE procedure dictionary and this will not affect actual diet
orders, as nourishments can have a much broader range and frequency of
change.

We only enter diets in the Diet procedures. We have a supplement
category where we enter supplement orders, and a category for other
Nutrition orders such as consults, and Admission screening referral
orders. We call it NUTRCON category. We use it for physician ordered
Dietary education, Dietary consults, etc.

We do something similar, we only order Diets via the Diet Category and
we use another category for supplements and consults, DSN (Dietary
Special Needs).

We too have a separate category for diet consults. Supplements are
entered in a query on the primary diet order via query Special
Modifications. We have a similar problem with NPO though. We don't use
concurrent diets or holds. NPO is entered as the diet order. We
sometimes miss the details when the diet order is resumed. They enter the
primary, thus cancelling the NPO, but forget the supplements or special
modifications.

We do have this same problem, so we just created another diet order
called "Add to Current Diet." See screen shot. That way the staff in the
dietary department knows not to cancel main diet order but to add what
is written to it.
