Hi Denise
We use one intervention only for restraints and change directions as needed.
The staff do initial charting there, as well as q shift or whatever is
needed, and then also end of restraint charting.

How are you charting restraints? Do you have one restraint intervention
that you use to document on all restraints, and change the directions
depending on what restraint you are using? Or do you have several different
restraint interventions with assigned directions? If so, how do you handle
it when more than one type of restraint is ordered? For instance, patient
may have wrist restraints or Geri chair. If you have 2 separate
interventions, both show up on the worklist. Do you make one inactive?
Only order one and make a note in the Edit Text? Other ideas???

We use only 1 intervention. We also only require documentation at the
end of the shift.

I am sending the screen shot. If you want to see our policy as well,
just let me know.

We decided to go this way because when we started, we were documenting
on paper every 15min. That seemed very unrealistic and we wanted to make
it as easy as we could. I know you aren't Joint anymore but we have not
had any issues with Joint over this since our policy clearly states the
expectations. I remember very well, some of my coworkers filling out
the paper restraint form for the whole shift at the beginning of the
shift. I didn't as I didn't know what I would do if my patient happened
to die before the end of the shift. Thought it would raise questions.

continued, discontinued or reapplied different sections of the screen are

I have attached our Restraint Flowsheet, Chemical Restraint Flowsheet
and the Siderails/Gerichair Flowsheet Also is a special Ventilator
Restraint flowsheet for our vent patients.
