We actually started scanning EKGs into Meditech 2 weeks ago. We wanted to get the Cardiopulmonary department very familiar with the process before we turned it over to the medical staff - expectation is "everything"!! Our primary reason for wanting them available in Meditech is for EKG comparisons. Our process is that we scan them into Meditech after they have been read (thus the interpretation is hand written on the image). The staff has done a really good job acclimating themselves to this new process. We'll see how receptive the medical staff is in a week or so!!!

We like the Ergatron arms that hold the monitor, keyboard and mouse for
most rooms. Some rooms are too small or we have "special" patients in
them and the computers can't be left in them. In those rooms, we have
cows that we keep outside the room. So far, no complaints about either
system.

You can run a report in BAR without creating a NPR report. Compile a
selection report with the following set up and this will pull the accounts
that have transactions posted to N.

Hi All
PHA users often use 'tallMANN' to help differentiate spell-alike/sound-alike drugs, this is also a safety requirement for some sites.
This shows nicely on Process Orders screens and printed forms.

However on Datamax thermal labels this has always been a problem as any increase in size automatically switched the 'tallMANN' to 'TALLMAN'.
Also I found the Datamax printers only responded to SIZE increases of 0.5 and did not respond to FONT=b

To my surprise this has changed. I am working with a new C/S 5.55 and new Datamax M Class Mark II printers
They now respond to FONT=B and to SIZE increases of 0.1 increments.
PLUS they now print any size/font in 'tallMAN'.

I don't know what changed at Datamax as Meditech has always told me this problem was beyond their control, but I wantred others who are planning to purchase printers to be aware of this.

Has anyone attempted to use Meditech's SCA product to scan EKGs into
Meditech? If so, what's the quality of the EKG in Meditech? Could
cardiologist actually read the studies from Meditech once scanned?

I am looking for some input on how other labs are working with reflex
testing done at a reference lab. We currently are adding on the test
and manually dropping in results. Is there an easier way to do this? I
have asked Meditech about this and they indicated that I would have to
send it as a profile with both tests and cancel if I did not need the
potential reflex test. Does anyone have a different process that they
would like to share. Thanks-hope you all had a nice Thanksgiving.

Debbie,

One solution would be to use the auto-fax feature. You would need to
have Telecoming set up for this. The other thing you might try is the
Call feature that is technically a part of Outreach. I have looked at
it, but not tried to implement so far. We mostly use faxing.

Happy Monday in L-ville,

I have a question on how your facility handles the following BBK scenario:

A patient is scheduled for surgery. How far in advance do you have them
come in for pre-op testing? What is ordered for pre-op testing for the
blood bank? Type and Screen? Crossmatches? Also, how is the patient
registered when they come for pre-op testing? PRE IN or REG REF or ???

Currently, our blood bank orders are handled through LAB, so it doesn't
matter how far in advance the crossmatches are ordered. The patient comes
in 2 weeks before the surgery for pre-op testing and is registered as a REG
REF. Then when they come in they get admitted per usual inpatient, so there
are 2 accounts. Is this how it is usually handled? I'm told this is
necessary from the billing side, so they can get prior authorizations in
place.

Thanks in advance for "schooling" me on this scenario.

There seem to be multiple ways the reflexed testing issue can be
handled.
What has to be looked at are the following:
1. Does the reference lab send back the results, including the
result(s) for primary test(s), and the reflexed test(s), all at the same
time?
2. Does the reference lab send back the reflexed test(s) even if they
were not performed. Usually with a N/A result to indicate that it was
not performed.
3. Or, does the reference lab send back the primary tests and just the
reflexed testing result(s) as they are performed?

If 1 and 2, you need to build all of the tests attached to the single
ordering panel/group test.
You can handle the reflexed test charges in one of two ways
1. Build all of the potential reflexed tests without
charge codes attached
and build charge tests and use reflex order
groups to add them to the specimen.
The reflex order groups will be triggered
using results or flags returned.
2. Build the possible reflexed tests that the
reference lab will send back and map the result
returned from the reference lab that a test
was not performed to TNP. (on pg. 3 test dict)

If 3, you can build the tests and reflex order groups to handle the
possible scenarios for the resulting of the test(s).

This is a quick run through and may not include all possibilities.

network folder for use when a planned or unplanned downtime event
occurs.

We are running both Meditech and LSS MAGIC 5.62. We are utilizing the
Caremedic Medicare RT and Claims Management applications for submitting
claims. We are in need of a way to automatically download (and if
possible ftp) our claim files. We use the auto close feature in Meditech
which is creating the claim files. Are there sites out there currently
doing this from both systems that can assist with setting this up?

When placing an order in pharmacy, you have the ability to change the
order type from IP to OP. Would anyone know what field this
information is captured in?
I've been asked to remove all OP orders on a report I created but I'm
drawing a blank as to how to capture it.

your own staff? If so, what companies did you use? How happy were you
with the results? Did you get knowledge transfer from the consultants
to your own hospital staff? Would you recommend using a consulting
company for something like this or do you think module operational
assessments from the Meditech consultants is a better way to go? Thank

You may want to check out the Call List functionality in Lab Outreach.
A specimen can be tagged to get on the call list when complete. Tag can be done at req, or at resulting. Techs would not have to do the calling.
This requires secretarial folks to periodically call up the list as a batch or paper, then process (call/fax) through it. System will log activity.
I have not had luck with folks to adopt this functionality, but it would reduce stat abuse for printing & faxing results out.

Hi everyone,

I'm hoping you can help me out with something.? We are implementing ITS in Magic 5.62 along with Amicas PACS.? The interface currently in place is sending the internal accession number over to PACS to track the order, but we are unable to get this value to print on the Requisition.? Along with this, the number does not help us in following the order back and forth between PACS and ITS since we cannot pull up orders in ITS by this number.? I was wondering how some other sites with RAD/ITS and PACS are handling this dilemma.? Any information would be extremely helpful!

We use and IV start assessment to document the start times and an
assessment to document on the stop. The stop query sends a notification
to Pharmacy that the IV fluids have been dc'd. This is for the orders
that say "lock the IV when taking PO fluids", since the orders to dc the
IV are in the orders that are faxed to Pharmacy.

to "How are you documenting IV Start and Stop times?" I did not see one

I think what Meditech means is the mail server that Meditech communicates with to send the messages. This is a mail server external to Meditech.
You will need to check the MOX settings to see who it is communicating with externally and see whether that server is still operational.

We are a Magic 5.6 facility that has fully implemented the Meditech
Magic Bed Board along with the Forward Advantage Bed Ready Telephony
interface.

Everything was working fine until sometime in the last month the OA
messages Meditech was sending to the internet addresses of alpha-numeric
pagers carried by Housekeeping staff stopped. (The messages are sent
to Housekeeping when the bed is vacated by a patient. The page alerts
them to go clean the bed for the next patient.)

We here at the facility have not changed any of the MOX mail settings,
the OA message set up or the Bed Board Housekeeping notification
configurations.

According to Meditech support (see the excerpt below), a problem with
the Meditech Mailserver Access is suspected.

HAS ANYONE ON THE LIST ENCOUNTERED AN ISSUE LIKE THIS? IF SO WHAT
RESOLVED THE PROBLEM?

Sorenson,Benjamin (MEDITECH) - Nov 24, 2008 - 1345 EST: Hi Joan, I had
programming dial in and check internally and it appears that the
Housekeeping notification messages (as well as my test messages)
internally indicate there is "No Mailserver Access". So either there is
an issue with the mailserver itself, or the mailserver does not have the
proper permissions. The programmer did verify that he can see that was
working recently due to the sequential email URNs within MEDITECH, but
again, something has changed with the mail server. Do you know if
perhaps a local user instead of an Admin user last signed onto the
server, because that can cause permissions issues. If you need
assistance with troubleshooting the mailserver, let me know and I can
put you in touch with my PC and Networking group. I will check back you
tomorrow to see how you are doing with this. Thanks, Ben

Hi Tim,

It is common practice to lower AV scanning on 'select' folders to ensure program stability. For example, you should never scan your SQL DB files as it can interfere with SQL. As an example here is the shortcut to the Microsoft SQL AV Scanning best practices one: http://support.microsoft.com/kb/309422
Do a google search on AV scanning best practices if you need more info on what to scan and what not to.

We have a number of products within our environment where the vendor specifically requests select files/programs/folder be excluded - this is common and is always one of the project questions as part of a new rollout (that and what to backup). Since viruses (todate) do not target specific non-microsoft products the risk is low.

Remember, the AV software is still monitoring for active viruses in memory so if a virus kicks in it will still catch it, it just isn't actively scanning the folder noted.

We are a MEDITECH 5.62 site in Indiana and they are installing PFS and
Clinicals (EDM, PCS, etc). We also use Citrix

We also run MEDITECH with a Windows 2003 server with Active Directory and
McAfee Anti-Virus. MEDITECH is recommending that we turn off the active
antivirus scanning on the MEDITECH FS Folder in our network.

MEDITECH states that this should not make our network vulnerable to any
additional slammer virus's etc. MEDITECH's stated benefit is that it will
increase response time and decrease the amount of "goofy" error messages
that often arise in PCS, EDM

Here is our challenge: The IT Director doesn't buy this concept from
MEDITECH and is very reluctant to lower any antivirus scanning on the
Windows 2003 server. We were hit with a slammer virus last year and are
still reeling from this.

Does anyone have any thoughts or similar concerns, please share.

current user restrictions on end user mail box sizes, in terms of mb? We
are trying to survey other hospitals, to see how we compare in terms of

based on job title, etc??

We currently impose a restriction at 150mb per user - and it's an across
the board restriction for mail box size limits for all users.

I have looked through the archives and have only seen repeated questions
to ?How are you documenting IV Start and Stop times?? I did not see one
response. So here I go again ? Does anyone have an answer or at least a

Terry:

Here are our screens. The staff know to document on them and that it
affects charges. Since they had an explanation, we've had excellent
compliance per our nurse auditor. The IV record allows you to DC the
fluids without discontinuing the site (e.g.: putting in a lock); the IV
site allows you to enter that the site was discontinued.

We teach OE, PCI and MOX in a 6 hour class with a short competency.
Then we teach NUR and build on the NUR intervention via OE in 2-8 hour
classes. This includes a competency on the admission process and
building a care plan.
We often hear from new employees that we have a very comprehensive
orientation.

We don't use agency nurses. In the past, when we did, they were not given access to our systems unless they came to the hospital regularly. They were then given the same education and certification as staff.

Nicky,

We just went LIVE with the GE clinical interface for our 26 bed ICU. We are Magic 5.6.1.The ICU nurses use the status board for documentation. During the training for CMI, after they validate the vital signs, I instructed the nurses to use the critical flowsheet to document the rest of the information. The flow sheets make it easier for the nurses to document the cardiac rhythm, temperature, etc. If you would like a screen shot, please let me know,

Have you tried going to the field where users change the order type from
IP to OP and pressed Shift F9? This will tell you the DPM, segment and
field name for the field. In magic it's Shift F8 F8.

First off, macros are ALWAYS better than fragments, so you're on the right track. Think: "Fragments Suck."

There are definitely some minor issues with your code, including the fact that the patient subscript is never established. It looks like you're running the loop prior to knowing your target patient, so the way I would do this is:

This translates fine but I never seem to hit my debug, meaning I never
find a single record in RADRW. I'm not too familiar with prefixes in
Magic (c/s is just too helpful with this). So maybe it is my Z.link
syntax. Or because RADRW is $T, I should just give up and go with the
fragment?
