Happy Friday!

We use Dragon Naturally Speaking (Nuance) in conjunction with Peter Cohen Associates for physician documentation in Meditech. Nuance recommends to avoid corruption to voice profiles due to aggressive anti-virus software to exclude the following file types from scanning. Wondering if the Dragon users out there are following this? There is concern here applying these exclusions. Especially as we have Dragon on devices that leave the hospital. Just looking for some input.

Recommendations from Nuance/Peter Cohen Associates
You should exclude from the anti-virus scan any files found in the folders indicated below or with the extensions listed below.

Dragon Medical Client Workstations (Windows 7, XP, and Vista):

* C:\Program Data\Nuance\ folder and all subfolders
* C:\<Windows user_ID>\AppData\ folder and all subfolders
* C:\Documents and Settings\All Users\Application Data\Nuance folder and all subfolders
* C:\Documents and Settings\Windows user_ID\Local Settings\Temp folder
* Files with these extensions:
BD, BIN, DAT, DVC, ENH, GSB, GRM, GRX, INI, LCK, NWV, SIG, SVC, USR, VER,
VOC, WAV, XML, LOG
Thanks,

We use Dragon enterprise and have set the permissions as indicated by PCA/Nuance as we did have problems when we missed a few folders. Since we excluded the require folders we have had no more issues. There should be no issue with setting these folders to exclude as your real-time anti-virus scanner will catch any viruses incoming to the device. This is no different than a lot of software applications (Microsoft SQL comes to mind) that require you exclude certain areas as Anti-Virus interferes with the software operation.

I would be cautious on this one. The %Appdata% and the %Temp% folders
are where a lot data is dropped while under the user context. Web Pages
and any incorporated scripts come to mind. BIN files are binaries and
can be easily infected, INI files can change your system settings, etc.
Hopefully you have these users set up as restricted users and not
Administrators.

You could exclude the folder those files are stored on rather than specific
file types.

All our users are setup as restricted users; even our IT folks (they use their admin rights only when needed).

Following were the only exclusions setup:

a. All Files in the folder C:\Program Files\Nuance
b. All Files in the folder C:\Documents and Settings\All Users\Application Data\Nuance
c. All Files in the folder C:\Documents and Settings\[windows login account]\Application Data\Nuance

We also have layers of protection; One AV system for our users/desktop; a different AV system for our web gateways/controller/firewall and yet another system for our e-mail environment.
However, as we all know, in the AV world you are never totally safe (for example, a new virus not yet identified by any AV companies) so you do the best you can.
