We are Magic 5.61 too. We chart pupil assessments as part of a
Neurologic Observation Assessment. Staff chart on 2 pupil
characteristics:
1. Pupil Size using grouped responses including Dilated, Normal or
Pinpoint.
2. Pupil Reaction using grouped responses including Brisk, Fixed or
Sluggish.
