[MT-L] Inter-Agency Patient Referral Report W-10

We are a Connecticut hospital and the State of Connecticut Department of Social Services requires an Inter-Agency Patient Referral Report “W-10” to be sent with a patient when transferred to an Acute Hospital, Chronic Hospital, Nursing Facility, Home Health Agency or a Rehab Center. Has anyone developed one using Meditech Report Designer. We are on MT Version 6.05 PP13.  Thank You. Glen
