Greetings, L-listers,

 

 How have other sites backed up the electronic Medication Administration Record for printing when there is an unplanned Meditech downtime? 

 We are Magic 5.5 SR3.  Using Meditech’s FTP routine, we’ve been able to spool the MAR to a PC hard-drive but the report scheduling routine does not seem to allow hourly or semi-hourly scheduling.  How have others resolved the need for frequent backup?

actually, you can schedule for every minute (not that you'd want to). I
have a census report that is SCHED for every hour.
In the E/E Report Schedule window, use frequency = DAILY, then make
your MIS start times whatever is appropriate. You can add as many start
times as you need. (0100, 0200, 0300, 0400, etc.). I recommend that you
don't choose whole hours, since most people set their schedules to run
on the hour. Sometimes my little report got lost in the SCHED shuffle
and didn't get run when I had it set for the top of the hour. Instead,
I changed to 0055, 0155, 0255, etc to stay away from these peak times,
and now it never fails to run.

We have ours scheduled twice an hour.  Set the Frequency to "Daily".  Set the days to "SUN:SAT" and the time as desired, say 13:00.  Then to get the next run, in the days, repeat SUN:SAT and the next time would be 13:30.  Repeat this until you covered your 24 hour period.  The schedule gets long, but once its done its done.

We use Interbit’s Mprint/Netprint product to capture the MAR every hour.  Pretty slick.

Please post. We’re in the process right now of developing our downtime plans for e-MAR as we plan to go LIVE with it in the ED next month.  We plan to use Automate to script the e-MAR and some other reports to run and download every 30 minutes to an hour.  I’d like to know what others are doing.  Please post.

I have noticed that many of you have been looking for a way to backup your eMAR during scheduled and non-scheduled downtimes. Some responses I have seen answer some of the ways to back up the eMAR, but what happens if you can’t get to the network which is the most frequent cause of downtimes? It is a good idea to have the eMAR and other important reports backed up on your servers, but it is as important to have them backed up on a local hard drive so that they can be printed when the network is down and you can’t reach the main servers.

 

MPrint Server gives you the ability to define PCs and/or printers attached to PCs throughout your organization as MEDITECH devices. Printers can be shared even when they do not have Network Interface Cards (NICs) or terminal server ports. Reports can be automatically directed to PC hard drives via MEDITECH spooling utilities. We are LIVE housewide with eMARS.  We pull eMAR data from DR via a report.  It updates about every 10  minutes.  Nursing units have an eMAR downtime report icon mapped onto their desktops.
In the event of a planned or unplanned downtime they simply click on the icon and print their downtime eMAR.
Documentation is recorded on this report and then once we are back online they "catch" up electonically.
We also FTP an NPR report as a back up but it is much slower than DR.

Backing up the eMAR on the DR is a good idea, but does not solve the network downtime. If the network goes down, you will not be able to reach the eMAR on the DR. Mprint Server is independent of your network. With eMAR stored on a local PC in each dept/ward, even if the network is down you can print the eMAR to a local printer. Network downtime is a more common downtime then MEDITECH going down.

Actually, the Data Repository (DR) generates the eMAR in a nicely formatted Adobe Acrobat file and publishes it to PCs located at each of the hospital’s Units. The reports are updated on a 10 minute cycle, a frequency not possible using an NPR report.  The Unit PCs, along with a local printer, are connected to backup power (red outlets) making each Unit’s report available whether MEDITECH applications go down, the network goes down, or power goes down.  Each report not only provides eMAR administration documentation for the previous 24 hours but also projects forward 24 hours providing appropriate space for documentation of existing active meds as well as space for new meds to be ordered during planned or unplanned downtime.  

 

Using the DR running in continuous update mode has proven to be a very robust eMAR/BMV downtime solution and this strategy has been employed at several other MEDITECH hospitals.  Please feel free to contact Mike Grisanti or me if you would like additional details or to look at a sample of the downtime eMAR report.

     I've seen of late articles on using RFID to identify patients when doing BMV instead of wrist-bands. It looks like there are several advantages to using RFID over wristbands. Does anyone know if Meditech and scanners that work with Meditech are able to use this technology for identifying patients? Also, can you use RFID to identify the patients and with the same scanner, use bar-codes to identify a medications since it would be impractical to carry around 2 different devices.
     All of this arose when our hospital identified a problem with reading bar-codes on wristbands when scanning with an Accucheck device to identify the patient. We found that nurses were using work arounds and as a result, scanning the wrong patient into Accucheck, glucose results were being placed on the wrong patient, and the wrong patient was receiving insulin.
      I thought that if the hospital was going to adopt a new way of handling wristbands, that they should go to RFID if it is feasible at this time and not just in the experimental stage. Why waste money on an old system only to replace it with something better in a few years if the newer system is already available?
