Chayav Mitzvoth? Subject to ' S'char V'onesh? '

To What extent, is the Jewish psychotherapeutic client chayav Mitzvoth
and Limud Torah?
To what degree does Rambam's 11th principle S'char v'onesh apply?
Is his/her Torah learning Meaningful?

Does the therapist, or any human for that matter, have any power to
define this?
Or is it only HKBH who sees into the heart and does a cheshbon of
Kavannah and Free Will?

Do secular definitions of sanity/competence have hallachik credibility?

Is there a continuum of degree of chiyuv, like Rav Dessler's Free Will
point?
Or can an individual be declared 100% posul as a Yid and Human.

Is there a difference with Positive and negative Mitzvoth?
