Do not see the page attachment icons

Hi, 

I have installed this extension recently and I don't see the attachment icons. I have no JS error, cleared my browser cache and server cache and also make sure that there are two tables created in my DB for the page attachment.

Can someone please guide me on  setting up this? 

I am also logged in as sysop.
