Dear Students,

All are advised to collect the degree data form (format as attached) available with Ms. Priya in Acad. (Admin.).

Following points must be strictly noted before taking / filling the forms:

1. No extra form is available with us therefore all the students should carefully read all the instruction clearly mentioned in the form before start filing the form and should also ensure the complete care and diligence while filling the same as NO EXTRA FORM shall be given to any student under any circumstances.

2. Students should come along with Two/Three color photographs strictly as per the details / dimensions of the photograph mentioned in the attachment. Student should also ensure to come along with necessary stationery items like Blue & Black Pen, Glue Stick, Pencil etc. required for filling the form.

3. Duly filled-in, signed, clear & tidy and unfolded Forms may be submitted on the same day to Ms. Priya in Acad. (Admin.).
