Hello,

Healthcare workers, emergency service workers, education and childcare personnel and individuals age 65+ are invited to register for a Pfizer COVID-19 vaccination clinic at the College of the Redwoods Eureka Campus gymnasium at 7351 Tompkins Hill Rd, Eureka, CA 95501. For a map of the campus, please click here.  Signage will direct you on-site as well. Please be aware: no photos will be permitted at the clinic site. If you received the Moderna vaccine, please do not register for this clinic. Please use the Additional Appointment Request form below to request a Moderna second dose appointment.

In order to be vaccinated at this clinic, each person must register in advance individually using the link below. (Links are specific to clinic dates.) The link will collect the required registration information and will allow to select your appointment time. Please do not share this link. Abiding by these guidelines helps ensure that we can get vaccine to our highest risk settings before moving onto other groups. Please be aware that there may be legal consequences to falsifying one’s employment and/or risk category to receive a vaccine for which one is not yet eligible. COVID-19 vaccine cannot be offered to those under 18 years of age at this time.

If you are currently under quarantine or isolation orders, please do not attend a vaccination clinic

If you are aged 65 or older, please bring proof of date of birth with you to your appointment. Examples of proof of date of birth include:

  • A drivers’ license, passport or ID card
  • Birth certificate
  • Any official document showing your name and date of birth matching a photo ID

If you are part of the workforce, please bring a photo ID and proof of employment with you to your appointment so staff can verify your eligibility to receive vaccine. Examples of proof of employment include:

  • A name badge or ID card from your employer
  • A paystub with your name and your employer’s name
  • A letter from your employer verifying your employment status (should include your name and be printed on your agency’s letterhead, if possible)
  • A printed or electronic copy of the email from your employer inviting you to participate in this clinic (must come from an organizational email address, not a private email address)

Registration Instructions: The registration link below will only work in Chrome and will not work properly with other browsers. It is recommended that a laptop or personal computer is used when registering. Registration using a cell phone is not recommended.
