Maximum Spin Hole Diameter for Dish End

Dear Experts,
 
As per ASME section VIII Div I , UW-34, max. allowable patch diameter for a spin hole of dishend is 60 mm.
 
If, the dish end spin hole patch diameter is found to be of more than 60 mm then the dish end should be rejected, please correct me if I am wrong.

At the time of forming the spin hole dia was less than 60 mm around 30 mm, but while forming the crack was observed at spin hole, so the hole was extended to 70 mm & the patch was welded. RT is also done for patch weld.
 
But as per Section VIII Div I, is it acceptable ?
