Observed crack after root bend of SA312 Type 316L

Dear Experts

I had faced a problem while doing the Root bend testing for PQR, Observed crack (image attached) after root bending, having such crack inase of SS316L pipe (3/4"xSCH160) is not common. Parameter kept during welding is as below:

Ampere- 90-110
Volt- 10-12
Shielding-10-12 LPM
Interpass-175 Deg C

Please share with your experience, what would be the reasons behind this failure?
