Tube to tubesheet pull out test load calculation

Dear Experts,

Please explain following
1) How to calculate Tube to tubesheet pull out test load as per ASME Sec VIII Div.I Nonmandatory appendix A. Pls. provide pullout test load calculation of tube to tube sheet weld with different joint design
2) What is difference between fr (Test) & fr (No Test) & how to select Joint Type as per Table A-2
3) Efficiency Value given in Table A-2 is minimum or maximum
