The problem may be due to the followings....
1. improper rolling of tube to tube sheet
2. improper cleaning of tube sheet chamber
3. improper setting of tungsten rod with the weld head
so, check the thinning percentage of tube with the tube sheet, ensure
the cleaning of tube sheet & tube & try to weld some mock-up pieces
for ensuring the proper working of weld head.
also check the weld overlap at the end of weld. the excess over laping
of weld can also make such a problems.
