From the attached WPQ record, it clearly indicates,
Test A was welded with 2”NPS x 5.54 Thk & Test B was welded with 4”NPS x 13.49 Thk.
Test A was welded with GTAW 2mm & 3.54mm SMAW , So his qualification is GTAW up to 4 mm & SMAW up to 7.08 mm, and Dia qualified 25mm & Over – Example WELDER X
Test B was welded with GTAW 2mm & 11.49mm SMAW , So his qualification is GTAW up to 4 mm & SMAW up to 22.98mm, Dia qualified 73mm & Over - Example WELDER Y
So,
GTAW 2mm only deposited in either case and its qualified up to 4 mm only, Not 8 mm.!
SMAW 11.49mm only deposited and its qualified up to 22.98 mm only, Not unlimited.!
QW-452.1 (b) Note (2) shall be used when the diameter restriction along with thickness limitation comes in pics.
Example, In your case Test B i.e. WELDER Y cannot weld 25mm Dia pipe.

I have scrutinized your below query and its comes with conjunction to CASE A acceptable and CASE B not acceptable, please correct me / advise me if i am wrong.
CASE 1)

With both collaboratively gives a result as per below.
Dia 1'' O.D / 12.20mm GTAW dep / 25.86 SMAW dep which is acceptable as per below interpretation of ASME section IX 1993 edition.

Background: The combination of welding
processes and pipe diameters listed below were used for performance
qualification:
(1) 2 in. Sch. 80 (.218) pipe with open butt
using GTAW process.
(2) 2 in. Sch. 160 (.343) pipe with consumable
insert and fill pass using the GΤAW process and the remainder welded with SMAW
process.
(3) 6 in. XXS (.864) using SMAW process with
backing.
Question: Using the combinations listed in the
Background, in accordance with QW-306, Note 2 of QW-452.1(b) to determine
maximum thickness qualified, and QW-452.3 for minimum diameter qualification,
is the welder qualified to weld on unlimited thickness and diameters above 1
in., using either an open butt joint or a consumable insert with the root layer
deposited with the GTAW process and the remainder deposited with the SMAW
process?

Answer: All are same as per CASE 1 except of SMAW noted as unlimited, which is not acceptable as ASME SEC IX QW-452..1 (b) states Thickness, t , of Weld Metal in
The Coupon, in. (mm) regardless of note 2, a single 13mm coupon requires over with a three layers.
Conclusion
Though note 2 speaks ‘’maybe applied to the production, which is fully depends upon the owners satisfaction, for above 2 cases can be accepted by them or even rejection with new qualification.
