What steps will reproduce the problem?
1. Attempt to patch Moodle version 2.4.3+ (Build: 20130405)
2. Patch fails with particular files.

What is the expected output? What do you see instead?

Hunk #3 succeeded at 3500 (offset -22 lines).

Hunk #2 succeeded at 879 (offset -18 lines).

What version of the product are you using? On what browser?

Moodle 2.4.3+ on Google Chrome. Hosted on CENTOS 6, apache, php 5.3.3

Additional Notes:

I was looking through the patch file and if it patches based upon line
numbers then there are some issues with the line number specifications. I
was comparing the patch file with the php files side by side and the line
numbers do not correctly correlate.
