All,

Prior to sending my FOIA request I read that if it takes longer than 2 hours the agency can charge for their time to pull together the information you request. I added a line to the request letter stating “not to exceed $25 in fees”. You may want to state a limit as well.

Try this form.  All you have to do is fill in the blanks, print it, sign and mail.  The instructions do not show up on the printed copy (hopefully, if I did Javascript correctly)
