Information to the participants
I ( Ms. R. Shangmi Moyon, M.Phil scholar) am undertaking a study on the knowledge and training needs of social workers in dealing with perinatal mental health problems ‘under the guidance of Dr. Md. Ameer Hamza, Additional Professor, Department of Psychiatric Social work, NIMHANS, Bengaluru, Karnataka. This form is intended to provide information regarding the study, hence you may decide your participation in this research study.
This study is basically looking for those qualified social worker specialized of any specialization with minimum 6 months experience of working in mental health area to assess their knowledge and training needs in dealing with perinatal mental health problems (For this study perinatal mental health problems are those mental disorders that occurs in mothers/pregnant women during the period from pregnancy planning to one year of child). The researcher for the study will be using questionnaires to assess knowledge and training needs among the participants which is developed and validated by the researcher.
If you are willing to participate in the study you would be requested to answer few questionnaires dealing with information about yourself, and assessing your knowledge and training needs in dealing with perinatal mental health problem which would require 20-25 minutes to complete.
The participants are not entitled for any incentive in this study. Your participation in this study is completely voluntary and you may decline to participate or to withdraw from participation at any time.

Undertaking by the investigator
The information collected from you in the form of questions and answers will be maintained with strict confidentiality. Only investigators who are part of the study will have access to the study records. Your name will be removed /deleted once the study is done. Prior, during or after your participation, you can contact the researcher Ms. R.
