Hi Amy,

Yes these workgroups can be shared with other community partners.  We have four groups: Access to Services, Chronic Disease and Healthy Lifestyle, Behavioral Health, and Social Determinants of Equity.  If you know of individuals who you think might want to participate in one or more of these workgroups, please feel free to share this information. They can contact me via email if they have questions.  

The priority areas that we will be working on can be found on our website:  http://health.macombgov.org/Health-CommunityHealthAssessment under the Priority Areas.  

At our first meetings in September, each workgroup is going to spend a little time brainstorming who else should be at the table, so if you missed anyone, or think of anyone else between now and the meeting, you can also bring those contacts with you.

Thank you for your interest in our Access to Services Workgroup! Our first meeting date has been set for Wednesday, September 6 from 10AM-12PM.  We will be meeting at the Macomb County Health Department (43525 Elizabeth St, Mt Clemens) in the Auditorium.

 

This workgroup will be focusing on the following items:

 

GOAL: Increase knowledge of Macomb County resources and services
Objective 1: Increase access to and awareness of affordable, healthy food and physical activity opportunities through educational programs and resources.
Objective 2: Improve the community’s capacity to obtain, process, and understand basic health information and services needed to make appropriate health care decisions and engage in health promoting behaviors.
Objective 3: Improve coordination of referrals and services in Macomb County Health and Community Service agencies.
Objective 4: Improve capacity and utilization of affordable, preventive, and integrated oral health services for under-served populations in Macomb County.

 

All information for our Community Health Assessment (CHA) and our Community Health Improvement Plan (CHIP) can be found on our website: http://health.macombgov.org/Health-CommunityHealthAssessment.  As we make progress on our CHIP these updates will be provided on the website.
