Greetings, sharing the attached from Elms Family and Community Services

 

Reaching out to inform you on a new Youth Opportunity we are offering at the Mount Clemens Public Library called Data Detectives Club. During this club, students in grades 5th-8th will have the opportunity to learn about diseases and STEM related skills through data collection and analyzation.

 

 

Parents must register their students by this Friday 5/30/25 as there are only 9 spots left! I have attached the flyer and registration form link in this email.

A general authorization for the release of medical or other information is NOT sufficient for this purpose (see § 2.31). The federal rules restrict any use of the information to investigate or prosecute with regard to a crime any patient with a substance use disorder, except as provided at §§ 2.12(c)(5) and 2.65.
