PDSOsmust provide all required evidence at the same time as they submit the petition update in SEVIS. School officials must ensure that all required documentation is ready at the time of filing the petition update.

Supporting documentation may be requested in the form of a request for evidence (RFE). Therefore, designated school officials (DSOs) should have the documents ready when they submit the Form I-17 update in SEVIS.


Schools must report to SEVIS only institutional accreditations and recognitions that allow them to offer the programs of study listed on the Form I-17. Program or department-specific accreditations are needed only if the institutional accreditation or recognition does not cover all programs listed on the Form I-17.


If your school does not hold an accreditation or recognition in a section, leave None selected. There are several situations when accreditations or recognitions are not held. They are described below:


Each section on the Accreditations and Recognitions page must have an option selected. If your school does not have an accrediting agency or recognition, None is an acceptable option, and depending on your responses, Not Required may be selected.


The Expiration Date is required and must be in the future, but cannot be more than 100 years from today. It cannot be in the future. It should reflect the start date of your most recent accreditation.


The State Recognitions agency or organization is the state educational oversight body under which your school is authorized or licensed. If your school has campuses in more than one state, there may be more than one state authorization listed in this field.


Changes will be saved when you navigate away from the page but will not be submitted until you click Submit under Page 6: School Officials in the top left corner of the Optional Comments page.


SEVIS only allows schools to select an accrediting agency once. Since some schools have different review dates for their individual campuses, schools should enter the accreditation validity dates associated with the school whose accreditation expires last.


Multi-campus schools may have different accreditations for the different locations. SEVIS does not provide the ability to enter such comments when selecting a specific accrediting agency. Schools must use the Optional Comments field on the Accreditations and Recognitions page to capture this information.


Select the accrediting agencies from the appropriate drop-down lists. If your campuses have different expiration dates, enter the Effective Date and Expiration Date per the instruction above. In the Optional Comments field, enter the name of the agencies, followed by the listing of SEVIS Campus Codes for the schools covered by each accrediting agency. The school Campus Code consists of the last three digits of the SEVIS school code.


Most people think of pins, plaques, and other awards when considering how to recognize the work someone has done. The presentation of these awards takes place at our Annual Meeting and Awards Ceremony. Typical awards given at this time include:


If your Girl Scout would like to write an appreciation card to her Girl Scout volunteers, choose from these cards. She can fill it in and give it directly to her favorite volunteers. Maybe the troop cookie parent was super helpful or one of the troop co-leaders made your Girl Scout feel confident enough to try something new. These fill-in cards will help your Girl Scout say thank you!


If you would like to receive a pin to represent the number of years you have been a member and/or a volunteer (in 5-year increments), please complete this survey and notify your local adult recognitions coordinator.


If you have been a troop leader for less than two years and would like to earn the Green & Growing award, you can download & print this card or get a hardcopy from your volunteer support specialist to track your training and participation.

Appreciation Pin

Nominate a volunteer you know who goes above and beyond working with two or more troops. You will need to have two other people, not related to the nominee, to write letters of endorsement.


Honor Pin

Nominate a volunteer you know who goes above and beyond working with two or more service units. You will need to have three other people, not related to the nominee, to write letters of endorsement.


Thanks I /Thanks II Badge

Nominate a volunteer you know whose service goes above and beyond, significantly impacting the entire council or the Girl Scout movement. You will need to have four other people, not related to the nominee, to write letters of endorsement.


Commitment to Excellence Award

Nominate a volunteer who has received the Appreciation Pin or Honor Pin and continues to go above and beyond working with that level of service and scope. You will need to have three other people, not related to the nominee, to write letters of endorsement.


The Juliette Gordon Low Spirit Award

Nominate a volunteer who has served for 25 or more years at a level above and beyond expectation for the role. You will need to have four other people, not related to the nominee, to write letters of endorsement.


An individual or group may be formally recognized through ceremonial recognitions (resolution, proclamation or citizen recognition award). The Mayor is generally invited to participate in these recognitions.


The Compass Award is a rarely presented recognition given to individuals, groups, or organizations whose collective contributions consistently went above and beyond expectations and have benefitted the citizens or the city significantly. Recipients of the award exemplify and embody a strong sense of volunteerism, civic pride or commitment to bettering the community demonstrated over time. They demonstrate exceptional leadership and service to the community, making Ogden City a better place to live. The Council determines who receives or is eligible for the award. There is no formal nomination or application process


During the City Council meeting, a Council member will read the ceremonial recognition. If it is a resolution or proclamation, a vote will be taken. The Chair will then invite the individual or group being honored to come forward and receive the recognition and be congratulated by Council members. The recipient(s) will then have the opportunity to comment and have a photo taken with the Council.
