HPR Launch Paperwork and other questions

Hi guys, new NAR HPR level 1 candidate here. Just some simple questions:

1. What type of paperwork is required for my certication launch? Is there any other forms besides "NAR LEVEL 1 HIGH POWER CERTIFICATION APPLICATION"? 

2. Proof of Membership? I have a PDF copy of my NAR membership, but what proof is needed to show I am a member of LUNAR on the lauch day? 

3. For the question on  the "NAR LEVEL 1 HIGH POWER CERTIFICATION APPLICATION" reguarding CP and CG calculations, is a rocksim file acceptable proof?

   You will enter your NAR number on your application.
4. I have a PDF copy of my NAR membership, but what proof is needed to show I am a member of LUNAR on the launch day?
   If you are a member, your name will be listed on the signin sheet. If not, you may pay the walk on fee or join at that time.

1. Yes. A known kit is also acceptable.
