Attached is the completed application form and constitution.
The application form gives the impression that a formal proposal "may" be included but is not a requirement, thus only for formality. Hence I did not write it. Let me know if you think we need it, I will draft the formal proposal.
Please complete and edit the constitution, I could not fill in every section yet.
Sorry I could not joint the fellowship at Dr. Lai's as my brother needs to go back earlier so we rescheduled the flight to morning...

Dear Adrianus,
Thanks so much for helping me out with this and for the fellowship group. I really appreciate your help. These details were given to me by the juniors (Mel & Isabelle) in their conversation with Dr Lai, and I spoke to them again 2 weeks ago.
Dr Lai shared this verse the last time, that he hopes the fellowship can be a place for support and encouragement. Hebrews 3:13 "But encourage one another daily, as long as it is called "Today," so that none of you may be hardened by sin's deceitfulness."
I have attached the application form, and also the constitution format. This is the "description" of the constitution: The constitution is just a general guideline intended to help you with the formation and running of your interest group. You may include details including general elections, meeting times or places, members, or other relevant details. A sample constitution guideline is available : Student Interest Group Constitution Format.docx to help you with the drafting of the formal constitution. Constitutions need to be submitted within three months of submission of the initial paperwork. The three month period is granted so that your interest group has more time to gather members and officers and draft the constitution together.
The constitution layout looks more complicated than what we talked about haha although i think we will not need to fill out everything. Adri, do you think you can put together the purpose and mission in words? The other details we can raise up and talk about it when we meet at Dr Lai's house. After all, the constitution only needs to be submitted 3 months after the application is submitted.
Some questions:
1) Name of organization, will it be luke in duke? or we can have a generic name like duke-nus christian fellowship or varsity christian fellowship (<-- i think dr lai suggested this)
2) We need a president and treasurer for sure, plus one more position. i was proposing we should include year 1, 2, and 3 rep into the "exco" also. But we can overlap the duties for e.g. treasurer can also be a year 2 rep.
