Fillable PDF form in Latex

I want help in this form. I have made the radio buttons to select something but they appear automatic selected not by choice.
Please help me How to do it.

Can you host your code somewhere and give here the link? That would be
better than assuming what you are doing.

Can you host your code somewhere and give here the link? That would be

same names for each field.

Open and Download your pdf in cromium and save it, Then try to fill
Radio button. It will work

Yes, Di You are right.
This PDF is working in chromium browser. Even as Today Sir said, I
tried it in Evince Reader also but not working.

But we can't save it after filling it in Chromium/Chrome browser. And
in evince, it's not letting me choose the radio button, but we can save
the filled PDF using evince. You may try out something else to get
radio buttons work else we have the drop-down menu option that works
fine.

   Yeah, but if we go for radio buttons they are only working in chromium not in evience. And I will work on how to save the pdf.
