Adding Sitescope monitoring in PC

Friends,

I am using Sitescope 11.2 and ALM PC 11.52. We have created monitors in site scope and would like to add those monitors in PC. 

In PC, I selected Monitor Profiles and then selected Sitescope. I gave Server as "sitescope.xxxx.com", port I tried with 8080 and 8888 (The URL for Sitescope is http://sitescope.xxxx.com:8080/SiteScope/Srvlet/Main.

When clicked on Get Counters, I got error. So I selected "Use Account" check box and it enables three text boxes. Of the three, one is for userid, 2nd id for password and not sure what is the third text box. I am getting the error like "Failed to retrieve measurements. Please check the machine name, credentials and other supplied details and try again. If the problerm persists, contact your administrator". I tried providing userid/password in 2nd and third text boxes. Could you some one guide me in this?

Could some one guide me what details I missed?
