Cable outlet covers/grommets/trims

Not sure quite how to describe what I'm looking for.  I've got a few places where I've got a cable coming out of the wall through a badly finished hole - mostly speaker cables but a few other things.  Yes, I could have put on a backbox, a front plate with terminals, etc etc but that's a load of expense and extra connections and in some places there simply wasnt space.

What i want is something like these radiator pipe trims but for cables - 

Any suggestions?
