Recently I applied a new compliance policy to our corporate iPhones. This included the requirement that the passcode be changed occasionally. The time came to change my passcode. If you tried to use a password 3 times and failed, you are locked out As it stands right now, your phone is bricked and you can not even do a factory reset.. Your only recourse is to either call Verizon customer service or go to a Verizon store where they can enter in a special code (PUK) for your phone. This will unlock it and make it useable again. As you found out, worst thing anyone can do is forget their password.

I am understanding that you are locked out of your phone and not the sim. If this is correct then simply perform a hard reset. This will let you boot into hboot and perform a factory data reset. Instructions for a hard reset can be found under support on Verizon's website.

You can still reset it so no worries. You have tu turn the phone off and find out the button you needto press. I would google your phone model hard reset and it will be online. Its easy. Bad part is EVERYTHING wil be wiped.. EXCEPT memory card of course. PUK code does not apply here since its not asking for it.
