[PATCH] More consistent handling of milliseconds

Hi,

There appears to be an inconsistency in the way in which milliseconds
are handled. In some cases they are output as microseconds.

The attached patches improve consistency of millisecond handling:

02-fix-millis.patch: Forcibly truncate the microsecond value returned by
erlang:now() to milliseconds.

03-format-millis.patch: Ensure that millisecond values are padded to 3
columns.
