question about modular curves

Hello,

I tried to look up the explicit equation for the modular curve X_1(5) and for the quotient map X_1(5) --> X(1).  On 

   https://beta.lmfdb.org/ModularCurve/Q/5.24.0-5.a.1.2/

I found the equation for this quotient map in terms of "x,y" which I presume are coordinates of an affine equation for X_1(5). But the LMFDB page does not seem to have the actual equation for the curve.  I have sent along an annotated screenshot explaining what I have in mind. Many thanks for your help!

This is a genus 0 curve with a rational point, hence isomorphic to P^1, which we parameterize as (x : y).  You can plug in x=t, y=1 f you would prefer the map to be in terms of a single parameter t.
