I can't speak too much myself on the difficulties of dealing with monad transformers - but it's always nice to hear that PL folks take usability seriously :)

Atomo (a python/ruby-esque dynamic language embedded within Haskell) is an interpreter which uses Monad Transformers in the core, so reading through that might speak to the issue:  http://atomo-lang.org/ probably most relevant here  http://darcsden.com/alex/atomo/browse/src/Atomo/VMT.hs
