Help with REDCap Test /Exam.

My institution requires users pass a test on REDCap before being granted full access. I have been trying to take the test, and have consistently failed to attain the passing grade (even though curiously, I have used REDCap and other systems to design studies and study instruments).  

The test administration system has blinded me to the correct answers, so it is not clear which of the tests I am getting wrong - it is manually graded.

Before approaching my institution about the test and its answers, I am pasting my last iteration of the test I took.  I indicated the answers I selected with asterisks. 

I would be very grateful if you could let me know if some of the answers are actually wrong - and hopefully point me in the correct direction.  I have watched the REDCap videos, read manuals, hence I am not sure what I might be doing wrong.

QUESTION 1: In REDCap, a data entry field can be:

[REDCAP can export to EXCEL, SPSS and SAS. Is E a good choice)

These are the items I would have answered differently:

#3: a - A patient/study participant would need to authorize use of his/her PHI. This cannot be waived by an IRB.
#4 d -The question is about other data entry fields in general, not just those that would be ideal for text.
#6 b -Only changes that could potentially lead to a loss of data, such as changing the name of a variable,for example, must be approved by an administrator. If you did something like add field note, this could be implemented immediately.

I think that for number 3, the answer is "both A and B". It seems that in limited circumstances an IRB can waive the authorization requirement. See the section "Waiver or Alteration of the Authorization Requirement" here: https://privacyruleandresearch.nih.gov/pr_08.asp
