How to stop multiple responses by same participants? public survey

I am sending a red cap public survey for asthmatics. How can I stop same participants to submit multiple responses? Is there a function like recognizing IP address. I am not using email, it will be posted in different forum, and anyone can answer. Thanks in advance for your help.
