Required parameters for ISV's Own Site Acquire License Key HTTP request

What are the required parameters for an ISV's Own Site Acquire License Key HTTP request?

The only documentation I can find is at http://www.licmax.com/developers.html#isvAcq_license

On that page is lists the following request parameters:

prodDefKey

prodInstKey      

isvKey      

orderId      

qty      

       

From the example image it list the following parameters:

deviceId
orderId
qty
InstKey
prodDefKey
isvKey

However, the example ends with a "&..." so I assume there are more.

When I try to use just the above listed parameters in a simple HTTP request from a browser URL I get an Access Denied error page.
