New release

Changes include:
Added static/dynamic/const_point_cast for poet::monitor_ptr.
Added new poet::monitor template constructors which forward arguments
to the constructor for the wrapped value.
Fixed problems caused by default copy construct/assignment operator in
poet::monitor.
Fixed monitor::scoped_timed_lock constructor.
Added new code to reuse old nodes of the acyclic_mutex locking order
graph, to limit its growth when many acyclic_mutexes are continually
default-constructed and deleted.
