Need Form for Policy Assignment under (LOVE & AFFECTION )

Dear Friends !

Can anybody arrange the Form from Policy Assignment under LOVE & AFFECTION...

In our Branch PS Department,Neither Form is available nor they don't know which form no. will be used for Policy Assignment under Love & Affection..

if possible, kindly arrange ...

1) Form No.3855 - Form of Notice of Assignment.

2) Form No.3846 - Form of Absolute Assignment by way of gift in favour of one assignee.

Stamp duty is not required if the assignment is done on the back of policy bond.  If it is done separately by way of deed, stamp duty is required.

There are assignment forms also available.  Depending on which form you require, it may used.

The necessary forms are available with Forms Manual, PS Department, LIC of India in any branch or on the website.
