Special MHR Form 3251 A & B Requirement

Dear Friends,
As per the latest circular dated 05/12/2016, it has been decided to increase requirement of Special MHR Form for SUC above 50 lacs (nil upto 50 lacs as per attached circular). I have recently come to know from my branch that if term rider has been taken in a policy & SUC exceeds Rs 25 lacs, Form No 3251 A & B will still be required even though SUC is well below 50 lacs. Is it correct? Plz confirm. If any clarification or circular in this regard has come, kindly provide the same.

Thanks. I have the NEW FORM while you have sent the OLD form. My enquiry is not the form but when it will be required & when it will not be. Meanwhile, I had to chance to enquire from my Divisional Office & they have confirmed to me that the form is required ONLY when SUC is above Rs 50 Lacs (not upto 50 Lacs).
