Form 680 DGH

Dear Friends,

Sorry forgot to attach the 2 page - DGH Form 680 in my last email.

PFA 2 pages - form 680 DGH. Revival of a lapsed policy can be done in any branch. But this rule is not followed in our 71X - Alandur, Chennai Division I, South Zone Branch. There is a circular for this. But I dont have a copy of it right now.  LIAFI should do something in making employees to implement this any branch revival ( wherever edms image of the proposal docs are available ).

Thank you very much Veera.

Since client is in Bangalore and his policy service branch is at Mysore. In Bangalore they are not accepting to revive.  I was able to revive one of our client policy in Mysore-1 Branch one year back but they asked me to show the client but finally able to convince Manage  showing the client through video calling and mail by sending selfiee photo..

We need to have a simple solution to the clients because many people keeps changing their job / moving one city to another which helps them to revive the policies.

Now all scanned images available online one should able to get the service at any branch. Only few branch managers will able to accept the changes and take decision and co-operate but not all.

What happens when a policy is lapsed and the policyholder is abroad working onsite for a company.  What is the procedure for revival of the said policy.

If Nonmedical case,  DGH Send to Policy holder and bring with his signature and witness signature, witnesse should be there Resident
