Data Mismatch NACH rejection

Hello,

I have received following SMS for NACH rejection, on asking the branch staff they are not aware of what data Mismatch means. On checking the nach form again all details were accurate and also clients sign in bank records is same.

Can anybody tell me the probable reason for rejection?

NACH mandate of Policy no. Xxx is rejected by customer bank for reason Data mismatch with image .Pl contact customer to submit fresh mandate immediately.
